摘  要:目的:研究免疫性血小板减少症(ITP)患者Th17细胞数量、相关功能分子水平及其与临床指标的相关性,探讨Th17细胞在ITP发病机制中的作用。方法:ITP患者40例,正常对照组14名。应用流式细胞术(FCM)检测骨髓IL-23R+CD4+/CD4+细胞比例;应用ELISA检测骨髓上清液IL-17、IL-23水平;RT-PCR测定骨髓单个核细胞中IL-17 mRNA的相对表达水平,并与临床指标进行相关性分析。结果:ITP患者骨髓中IL-23R+CD4+/CD4+细胞比例[(4.933±6.162)%]、IL-17水平[(13.434±7.189)ng/L]、IL-23水平[(56.930±58.512)ng/L]、IL-17mRNA相对表达水平(0.921±0.196)均明显高于正常对照组[(0.897±0.803)%]、[(4.134±2.879 ng/L]、[(17.841±11.033)ng/L]、(0.749±0.136)(P<0.05);活动期ITP患者IL-23R+CD4+/CD4+细胞比例[(6.699±7.008)%]、IL-17水平[(16.021±7.114)ng/L]、IL-23水平[(72.474±64.835)ng/L]、IL-17mRNA相对表达水平[0.967±0.186]均明显高于缓解期患者[(1.656±1.286)%]、[(7.972±3.326)ng/L]、[(24.114±17.654)ng/L)]、(0.801±0.163)(P<0.05)。ITP患者上述指标与其临床指标存在相关性。结论:Th17细胞在ITP患者免疫发病机制中发挥重要作用,在ITP患者中不仅数量增加,而且功能活跃。Objective: To study the quantity, related functional molecule level of Thl7 cells and its correlation with clinical parameters in patients with immune thrombocytopenia, and to explore the role of Thl7 cells in the immune pathogenesis. Meth6ds: Forty patients were divided into two groups, including 26 ITP patients in activity, 14 patients in remission. There were 14 normal controls. The ratio of IL-23R+ CD4+/CD4+ ceils in bone marrow of patients were measured with flow eytometry, and the level of IL-17, IL-23 in bone marrow supernatant was obtained using enzyme-linked immunosorbent assay while the relative expression levels of IL-17 mRNA in bone marrow with reverse transcription-polymerase chain reaction technique. The correlation analysis on clinical indicators of ITP patients was made. Results: The ratio of IL-23R+ CD4+/CD4+ cells[(4.933±6.162)%], the level of IL-17 [(13.434±7.189) ng/L], IL-23[(56.930±58.512) ng/L] and IL-17 mRNA [0.921±0.196] in bone marrow with ITP patients were significandy higher than those of normal controls[(0.897±0.803)%], [(4.134±2.879 ng/L], [(17.841±11.033) ng/L], [0.749±0.136](P〈0.05). These four indicators in active ITP patients [(6.699±7.008)%], [(16.021±7.114) ng/L], [(72.474±64.835) ng/L], [0.967±0.186] were significantly higher than those in remission[(1.656±1.286)%], [(7.972±3.326) ng/L], [(24.114±17.654) ng/L)], [0.801±0.163] (P〈0.05). The above parameters of ITP patients were correlated with clinical indicators. Conclusion: Th17 cells are very active with increased numbers in ITP patients, which plays an important role in the immune pathogenesis of ITP.
