早期经皮穴位电刺激结合康复宣教对脑卒中后抑郁发生率的影响

摘　　要：目的:观察经皮穴位电刺激结合康复宣教对脑卒中后抑郁(PSD)临床发生率的影响。方法:将180例符合纳入标准的脑卒中患者分为经皮穴位电刺激结合康复宣教(结合组)、经皮穴位电刺激组(经皮组)、和对照组,每组60例,3组患者均进行常规神经内科对症治疗,均不使用抗抑郁药物。3组患者在治疗前和治疗结束后使用汉密尔顿抑郁量表(HAMD)进行评分,并分别计算3组患者PSD的发生率。结果:结合组、经皮组和对照组的PSD发生率分别为8.0%,16.7%,36.7%;与经皮组、对照组相比较,结合组PSD的发生率差异均有显著性意义(P<0.01),经皮组与对照组比较差异有显著性意义(P<0.01)。结论:早期穴位电刺激结合康复宣教、单一经皮穴位电刺激对PSD抑郁的预防简便、安全、易行,但以前者对降低PSD的临床发生率更具有较好的效果。Objective：To observe the effect on clinical incidence of Post -stroke Depression （ PSD） intervened by transcutaneous electrical acupoint stimulation （ TEAS ） with propaganda and education of rehabilitation . Methods：180 cases with stroke met the inclusion criteria were randomly divided into the TEAS combined with propaganda and education of rehabilitation group （ the combined group ） , the TEAS group and the control group .There were 60 cases in each group on the basis of symptomatic routine treatment of neurology without an-tidepressants.The PSD incidence was evaluated by Hamilton Depressive Scale （HAMD） before and after the treatment.Results：The PSD incidences of the combined group , the TEAS group and the control group were 8.0%, 16.7% and 36.7% respectively.After treatment, compared with the TEAS group and the control group , the PSD incidence of the combined group was lower , with a statistically significant difference （ P 〈0.01）.There was a significant difference between the TEAS group and the control group （P〈0.01）.Conclu-sion：It is simple, easy, convenient and secure to prevent PSD by TEAS with propaganda and education of reha-bilitation or single TEAS.Moreover, there is a better effect on reducing the incidence of PSD by TEAS with propaganda and education of rehabilitation .
