2型糖尿病患者心血管危险因素与臂踝指数分布关系的研究

摘　　要：为探讨我国2型糖尿病患者臂踝指数(ABI)水平与心血管危险因素间的关系,对安徽省6个城市的1 121名(男性:671名;女性450名)2型糖尿病患者进行横断面研究。记录心血管疾病的症状史,记录血压和人体测量结果。使用超声多普勒血流测量仪测量臂踝指数。根据受试者臂踝指数≤0.9和≥1.30分为高和低臂踝指数。女性的BMI更高,臂踝脉搏传导速度也更高(P<0.001),而男性有更高的舒张压和II糖尿病病程更长。两性间的收缩压和臂踝指数的差异没有显著性。男性低臂踝指数(<0.9)患有率是4.47%,女性为4.67%。男性高臂踝指数(≥1.30)的患有率是14%,女性为10.45%。两性的年龄、BMI、臂踝脉搏传导速度和血压与臂踝指数值显著相关。臂踝指数可作为我国2型糖尿病患者心血管危险因素的强烈预测因子。The aim of study is to research the association between the levels of ankle-brachial index （ABI） and cardiovascular risk factors among people with type 2 diabetes in China. A cross-sectional study was carried out at 6 cities in Anhui Province on 1 121 subjects （671 males and 450 females） with type 2 diabetes mellitus. History of symptoms linked to cardiovascular diseases was noted, and blood pressure and anthropometric measurements were recorded. Ankle-brachial index （ABI） was measured using ultrasonic Doppler blood-flow detector. Subjects with ABI ≤0.9 and ≥ 1.30 were classified as having low and high ABI, respectively. Females had a higher BMI and brachial-ankle pulse wave velocity （P〈0.001）.Whereas, males had higher diastolic blood pressure and duration of type 2 diabetes mellitus. The differences of systolic blood pressure and ankle-brachial index were not found significant between the sexes. The prevalence of low ABI （〈0.9） was 4.47% in men and 4.67% in women and high ABI （≥ 1.30） was prevalent in 14% of men and 10.45% of women. Age, BMI （body mass index,BMI）, baPWV （brachial-ankle pulse wave velocity,baPWV） and blood pressures were significantly associated with ABI value in both sexes. The ABI might be used as a strong indicator for cardiovascular risk factors in type 2 diabetic subjects in China.
