我院帕瑞昔布用于围手术期镇痛的使用情况分析

摘　　要：目的分析我院帕瑞昔布用于围手术期镇痛的使用情况。方法采用回顾性调查方法,对我院100例普外科、骨科和妇科围手术期使用帕瑞昔布患者的镇痛药物使用情况进行分析。结果 100例患者中,普外科、骨科和妇科的比例分别为7%、93%和0。术前镇痛药物的使用比例15%,且只有5例联合使用镇痛药物。术中和术后联合应用镇痛药物患者分别为61和38例。帕瑞昔布在术前主要用于原发疾病的治疗;术中帕瑞昔布与阿片类药物联合应用的患者有59例;术后单独使用帕瑞昔布的患者为62例、与阿片类联合应用为13例、与非阿片类联合应用为25例。患者术后使用术后自控镇痛(PCA)泵的比例为69%,PCA给药均为静脉途径,品种主要为吗啡注射液。术后镇痛药物不良反应的发生率是4%。结论我院帕瑞昔布主要用于骨科手术的围手术期疼痛治疗,术中多与阿片类注射剂联合应用,术后则多为单独使用。OBJECTIVE To analyze the use of our hospital parecoxib for perioperative analgesia. METHODS One hundred patients of used parecoxib in postoperative in general surgery department, orthopedic department and gynecological department were an- alyzed by retrospective survey method. RESULTS The proportion of general surgery, orthopedics and gynecology were 7% , 93% and 0, respectively. The proportion of preoperative analgesic were 15% , and the combination of analgesic drugs were used only in five patients. But the number of intraoperative and postoperative combination analgesic were 61 and 38, respectively. Parecoxib was used for treating primary of preoperation ; In surgey day, the number of patients used combination of parecoxib and opioid were 59. After surgey there were 62 patients that only used parecoxib; in addition, combined with opioid and non-opioid were 13 and 25, respectively. The proportion of the use of PCA was 69% , and morphine injection was used mostly for PCA. The incidence of adverse drug reactions was 4%. CONCLUSION In our hospital parecoxib is mainly used for perioperative surgical pain treatment in orthopedic, and is combinedly used with opioid in surgery, but mostly used alone in postoperative period.
