PCT在老年人肺部感染中的价值

摘　　要：目的：探讨PCT在检测老年人肺部感染中的价值和应用。方法：回顾性分析2013年5月至2014年3月叙永县人民医院收治的145例年龄在65岁及以上的老年人患者的PCT检测结果，按肺部感染程度分严重感染组（38例），局部感染者组（85例），无感染者组（22例），并对各组PCT检测结果作对比分析。结果：感染组病人PCT浓度明显高于局部感染者组和无感染组（P＜0．05），局部感染者组PCT浓度明显高于无感染者组（P＜0．05）。在PCT的阈值为0．5ng/ml时，对老年人肺部细菌感染发生率的预测敏感度为75％，特异性为91％。结论：PCT能有效预测老年人肺部感染并判断感染的严重程度，是诊断老年人肺部感染特异性较高的标志物，检测患者血清PCT有利于早期诊断老年人肺部感染，判断病情严重程度，改善预后，减少抗生素的滥用。Objective：To study the value of PCT in detection of the elderly pulmonary infection and application.Methods：Making a retrospective analysis on PCT test results of 145 pa-tients of 65 and older age of elderly patients in People＆#39;s Hospital in Xuyong from May.2013 to March 2014 .According to the degree of lung infection serious infection group （38 cases）,local infection group （85 cases）,No infection group （22 cases）in detection of PCT and CRP level after admission.Results：Patients PCT concentration serious infection group was obviously higher than that of local infection group and no infection group （P 〈0.05）,and PCT concentration of local infection group was obviously higher than that of no infection group （P 〈0.05）.The PCT ofthethresholdvalueof0.5ng/ml,thesensitivityfortheforecastofsenilepulmonarybacterialinfectionincidencewas75%,specificityof91%.Conclusion：thePCTcanefectivelypredictthe elderly pulmonary infection and to assess the severity of infection,is the higher specificity of diagnosis of senile pulmonary infection markers,detection of serum PCT is helpful for early diagnosis of the elderly patients with lung infection and il ness severity,improve prognosis and reduce the abuse of antibiotics.
