米索前列醇、阿托品、利多卡因、安定对人工流产术的影响

摘　　要：目的观察米索前列醇、利多卡因、阿托品、安定在人工流产术前及术中用于软化宫颈、减轻疼痛,减少出血的临床效果及安全性。方法从2011年12月1日—2012年12月1日来医院接受人工流产的760例妇女中挑选出200例,血或尿HCG测定阳性,B超检查确定为宫内妊娠,均为初次怀孕。孕7-12周并要求终止妊娠者。将其随机分为2组,即观察组与对照组。观察组术前给予患者米索前列醇1片舌下含化,2 h后给患者阴道后穹窿塞1片米索前列醇,然后再给患者安定10 mg静脉推注,30 min后,用阿托品0.5 mg＋2%利多卡因10 ml对患者宫颈进行四点封闭并给予按摩,最后再进行人工流产手术。对照组术前不做任何处理,按常规方法做人工流产手术。结果观察组与对照组比较,手术时子宫颈软化及松弛程度、阴道出血量、手术时间、人工流产综合征发生率,差异均有统计学意义（均P〈0.01）。结论患者术前给予米索前列醇1片舌下含化,2 h后阴道后穹窿塞1片米索前列醇,然后再给患者安定10 mg静脉推注。30 min后,用阿托品0.5 mg＋2%利多卡因10 ml对患者宫颈进行四点封闭,手术时子宫颈软化及松弛程度较对照组明显提高、子宫收缩好、阴道出血量减少、人工流产综合征发生率显著下降。Objective Observation of misoprostol and lidocaine,atropine,stable in the artificial abortion preoperative and intraoperative used to soften the cervix,relieve pain,reduce bleeding of clinical efficacy,and safetyMethods From December 1,2011 to December 1 2012 since the hospital treated 760 cases of women abortion selected 200 cases,the determination of blood or urine HCG positive,B ultrasonic examination to determine for intrauterine pregnancy,were first pregnancy. Results Control group preoperative don＇t do any processing,according to the conventional methods do induced abortion operation. Conclusion Patients with preoperative give misoprostol contains 1 tablet under the tongue,2 h after vaginal fornix after 1 metre rope,and then to settle 10 mg patients with intravenous injection. After 30 minutes,0. 5 mg ＋ 2% lidocaine with atropine 10 ml of patients with cervical closed at 4 o ＇clock,cervix softening and relaxation degree when the operation was obviously improved,good uterine contractions and vaginal haemorrhage amount less,a significant reduction in the incidence of induced abortion syndrome.
