摘 要:目的探讨多层螺旋CT及其多种后处理技术对输尿管结石的诊断价值及其对临床治疗的指导作用。方法收集了2009年1月—2013年10月间经多层螺旋CT检查的输尿管结石43例,其中男性29例,女性14例,年龄17-73岁,平均41.7岁,回顾性分析其多层螺旋CT及后处理图像表现,并与临床结果对比。结果 43例患者共发现输尿管结石52粒,CT表现为圆形、类圆形或结节状高密度影,结石梗阻的输尿管近段增粗、扩张,CPR二维重建能直观地显示结石及输尿管扩张的情况。增强后肾盂期行三维重建即CT尿路成像能立体地显示输尿管结石及近段输尿管扩张等整体情况,图像立体感强。结论输尿管结石CT平扫后采用CPR,增强后采用CTU可显示结石和扩张的输尿管等整体情况,是目前诊断输尿管结石的最佳成像方法之一。Objective To investigate the value of multi-slice helical CT( MSCT) and post-processing technique in diagnosis of ureteral calculi,and its guidance for clinical treatment. Methods Total 43 cases of ureteral calculi by multislice spiral CT from January 2009 to October 2013 were enrolled in this study. There were 29 male,14 female,age 17- 73 years,average 41. 7 years old. The MSCT and post-processing films manifestations of 43 ureteral calculi were reviewed retrospectively,and compared with their clinical results. Results 52 ureteral calculi were found in 43 cases. CT finding of calculi was round,oval or nodular high-density lesion,and thickening and expansion of the proximal ureter caused by obstruction,CPR directly displayed calculi and ureteral dilatation. The three-dimensional reconstruction or CT urography in renal pelvis phase could show clearly the ureteral calculi and ureteral dilatation,and had strong stereo sense.Conclusion The CT scan,CPR and contrast-enhanced CTU can display the overall situation of ureteral calculi and ureteral dilatation,and is one of the best methods in the diagnosis of ureteral calculus at present.
