选择性贲门周围血管离断术治疗门脉高压出血的体会

摘　　要：目的：OK讨选择性贲门周围血管离断治疗门脉高压食管胃底静脉曲张出血的解剖基础、手术技巧及治疗效果。方法：回顾性分析行选择性贲门周围血管离断＋脾切除术治疗的31例肝硬化门脉高压食管胃底静脉曲张出血的临床资料。结果：所有病例近期均无再出血及肝性脑病，获得远期随访24例，1～3年内再出血1例，复查胃镜或上消化道造影的12例病例中有11例曲张静脉完全消失，1例明显改善，肝功能均有不同程度好转，血小板及白细胞均不同程度上升。结论：该手术治疗效果满意，是治疗门脉高压食管胃底静脉曲张出血的有效方法。Objective：To study the anatomical basis ,surgical technique and The therapeutic effect of selective pericardial devascularization and splenectomy on Variceal Bleeding in Portal Hypertension,Method： Clinical data of 31 patients of Variceal Bleeding in received selective pericardial devascularization and splenectomy were analyzed retrospectively.Result：no recurrent bleeding or hepatic encephalopathy took place in short term,The long-term follow up was performed in 24 cases,one case with recurrent bleeding in 1-3 years,the reexamination of gastroscope or Upper Gastrointestinal Imaging were performed in 12 cases, the gastro-esophageal varices disappeared in 11 patients, and 1 patients was obviously improved .All cases＇ hepatic function were improved in various degree, leukocytes and platelets were increased. Conclusion：the curative effect of this operation is satisfactory, this operation is an effective treat Variceal Bleeding in Portal Hypertension.
