硝普钠联合多巴胺持续静脉泵入治疗老年顽固性心衰临床观察

摘　　要：目的:探讨硝普钠联合多巴胺持续静脉泵入治疗老年顽固性心衰的临床疗效及安全性。方法:分析2012年10月至2013年10月采用硝普钠联合多巴胺持续静脉泵入治疗的40例老年顽固性心衰患者的临床资料。结果:治疗7 d后,患者呼吸、心率、血压、6MWT、心胸比、LVEF、LVEDD、LVESD及LA较治疗前均明显好转,差异有统计学意义(P<0.05)。显效27例(67.50%)、有效12例(30.00%)、无效1例(2.50%),总有效率97.25%。结论:硝普钠联合多巴胺持续静脉泵入治疗老年顽固性心衰临床效果显著,可有效改善患者临床症状,不良反应少,安全可靠。Objective： To investigate the clinical effect and safety of continuous intravenous pumping of sodium nitroprusside combined with dopamine in treatment of elderly refractory heart failure. Method： Clinical data on 40 elder patients with refractory heart failure treated with continuous intravenous pumping of sodium nitroprusside combined with dopamine from Oct. 2012 to Oct. 2013 were analyzed.Result： 7 days after the treatment,the respiration,heart rate,blood pressure,6MWT,cardiothoracic ratio,LVEF,LVEDD,LVESD,and LA were significantly improved when compared with those before the treatment,and the differences were statistically significant（ P〈0.05）. Excellence achieved in 27 patients（ 67.50%）,effectiveness achieved in 12 patients（ 30.00%）,ineffectiveness was in 1 patient（ 2.50%）,and the total effective rate was 97.25%. Conclusion： Continuous intravenous pumping of sodium nitroprusside combined with dopamine in treatment of refractory heart failure in the elderly has significant clinical effects,can effectively improve the clinical symptoms of the patients,has only a few adverse reactions,and it is safe and reliable.
