环乳晕小切口术与传统手术切除治疗男性乳腺发育症临床研究

摘　　要：目的:探讨经乳晕小切口治疗男性乳腺发育症的治疗效果及对术后美容效果的分析,寻找治疗男性乳腺发育症的最佳治疗方法。方法:回顾性分析2011年5月至2012年5月在我院就诊的18例男性乳腺发育症的患者乳晕小切口行手术治疗的临床资料,并且对术后局部外形、切口瘢痕及乳头乳晕等情况进行术后效果的综合评价。18例采用传统手术组的患者作为对照。结果:18例男性乳腺发育症的患者术后随访6个月,手术切口瘢痕不明显,乳头、乳晕无内陷,乳头感觉无异常可正常勃起,乳头及乳晕均显示正常,术后所有患者伤口均为甲级愈合,未出现感染、皮下气肿、血肿、乳头乳晕坏死等术后并发症,手术均获得成功所有患者对手术效果表示满意。相比较于传统手术组,环乳晕手术组在术后伤口愈合时间、瘢痕宽度以及乳房外观满意度等方面均显著优于传统手术组(P<0.05)。结论:经乳晕小切口治疗男性乳腺发育症的手术疗效较好,术后胸部外观及功能恢复良好,患者满意率高,适合在临床推广应用。Objective： To investigate the feasibility and effect on surgery of gynaecomastia by small circum-areola incision and its cosmetic effects by the purpose of finding best treatment of gynaecomastia. Method： A total of 18 patients with gynaecomastia from May 2011 to May 2012 treated by small circum-areola incision were retrospectively analyzed. The results were analyzed according to the postoperative appearance,feeling of nipple-areola complex and incision scar. 18 patients with traditional surgery were selected as control. Result： The operation was successfully performed in all 18 cases. Follow-up was done for 6 months,the surgical scars were obscure and the sensation of nipple or areola were good without retraction. There was no operation related complications,such as infection,subcutaneous emphysema,hematoma and all patients expressed satisfaction with the surgical results. Compared to traditional surgery group patients with gynaecomastia got less wound healing time,appearance of scar width and better breast satisfaction. Conclusion： The small circum-areola incision in the treatment of gynaecomastia is feasible and recommendable in terms of cosmetic maintenance and safety.
