关节松动术结合体外冲击波治疗肩周炎的疗效观察

摘　　要：目的:观察关节松动术结合体外冲击波治疗肩周炎的疗效。方法:70例肩周炎患者随机分为观察组和对照组各35例。2组均采用关节松动术治疗,观察组加用体外冲击波治疗。治疗前后采用视觉模拟评分法(VAS)评定疼痛,并评定肩关节疼痛与功能障碍指数(SPADI)及临床疗效。结果:治疗1及2个疗程后,2组患者VAS评分均较治疗前显著降低,且同时段观察组评分更低于对照组(均P<0.05)。治疗2个疗程后,2组SPADI亚量表及总评分均较治疗前明显降低(P<0.05),且观察组更低于对照组(P<0.05);观察组有效率明显高于对照组(94.3%、77.1%,P<0.05)。结论:关节松动术结合体外冲击波治疗肩周炎疗效显著,值得临床进一步推广、应用。Objective ：To observe the efficacy of mobilization combined with extracorporeal shock wave treatment in patients with scapulohumeral periarthritis .Methods ：Seventy patients were divided into observation group and control group randomly（n=35 each） .Both groups received mobilization treatment ,and observation group given extracorporeal shock wave treatment additionally .Two groups were evaluated by Visual Analogue Scale （VAS） ,shoulder pain and disability index （SPADI） and clinical effects before and after treatment .Results ：After one and two courses of treatment ,the VAS scores in both groups were decreased obviously as compared to pretreatment ,more significantly in observation group than in control group（all P〈0 .05） .After two courses of treatment ,the SPADI scores in both groups were decreased obviously as compared to pretreatment （P〈0 .05） ,more significantly in observation group than in control group（P〈0 .05） .The clinical effective rate in observation group was significantly higher than in control group（94 .3% vs .77 .1% ,P〈0 .05） .Conclusion ：Mobilization combined with extracorporeal shock wave treatment can alleviate the pain of patients with scapulohumeral periarthritis ,and the technique was worth expanding .
