摘  要:目的探讨儿童首剂麻疹类疫苗在8—12月龄及〉12月龄接种对其血清麻疹IgG抗体水平,评价麻疹类疫苗首剂接种月龄的选择。方法对2011年龙泉驿区人口麻疹血清流行病学调查项目中发现的首剂次麻疹类疫苗延迟接种(起始月龄〉12月龄)对象9例,与在8—12月龄合格接种首剂麻疹类疫苗的对象,分别以最后1剂次麻疹类疫苗接种时间距监测时间间隔(年)、年龄、性别、接种剂次、常规接种2剂次时间间隔(年)等条件,按照1:3进行配对,按《麻疹诊断标准》(WS296—2008)中规定的酶联免疫吸附试验(ELISA)间接法检测麻疹IgG抗体,应用EPI—INFO统计分析软件与IBM—SPSS20.0统计分析软件。结果麻疹类疫苗首剂接种起始月龄为8—12月龄与〉12月龄,其麻疹抗体显性感染保护率(Х^2=0.8,P=0.3711,α=0.10)和抗体水平(t=0.669,t0.10,(8)=1.860,P〉0.10,α=0.10)并无差别。结论将麻疹类疫苗首剂接种时间推迟至〉12月龄并不能提高血清麻疹IgG抗体的阳性率、显性感染保护率和抗体水平,同时考虑到暴露风险,不支持将麻疹类疫苗的首剂接种时间推迟到1岁。Objective To study the antibody level of measles vaccine in infants of 8 - 12 months and over 12 months for exploring appropriate vaccine age. Methods Nine participants with the first -dose vaccine inoculated over 12 months were chosen in the epidemiological survey of measles serum in Longquanyi district in 2011. Meanwhile, those inoculated in 8 - 12 months for the first dose were selected and matched with a ratio of 1 : 3 according to the interval of year from the last - dose, age, gender, number of vaccination dose and year interval of two routine vaccinations. The measles IgG - Ab of the serum were determined under Diagnostic criteria for measles WS296 - 2008 ELISA and the data were analyzed with EPI - INFO and IBM - SPSS20. 0. Results The apparent infection protection rate (Х^2 =0. 8, P = 0. 3711, α = 0. 10) and antibody level (t = 0. 669, t0.10,(8) = 1. 860, P 〉0. 10, α=0. 10) of the first -dose vaccine between those 8 -12 months and above 12 months were not statistically significant. Conclusion The antibody positive rate of IgG, apparent protection rate and antibody level could not improved by postponing the first dose vaccination to over 12 months old. Considering the risk exposure, there is no need to do so.
