经皮穿针微创治疗跟骨骨折35例效果观察

摘　　要：目的：探讨经皮穿针微创治疗跟骨骨折的的临床效果。方法：跟骨骨折35例,按Essex-Lopresti分型ⅠA型8例、ⅠB型6例、ⅡB型21例；按Sanders分型Ⅰ型14例、ⅡA型8例、ⅡB型8例、ⅡC型5例。均采用闭合复位克氏针内固定,术后定期摄片复查,了解骨折愈合情况。结果：手术时间25～45分钟，平均32.3分钟，失血量20～40mL，平均32.5mL。随访6～12月，平均8.5个月。功能评测采用Maryland足部评分系统，按疼痛和功能两部分，90～100分为优，75～89分为良，50～74分为可，〈50分为差；其中优23例，良9例，可3例，优良率91.43%。结论：经皮穿针微创治疗跟骨骨折手术时间短、失血量少、损伤小、并发症少、治疗效果确切。Objective：To explore the clinical effect of close reduction and percutaneous internal fixation by K-wires for calcaneal fractures. Methods： From April 2011 to June 2013, 35 calcaneal fractures were treated. According to Essex-Lopresti classification, 8 patients were Type IA, 6 patients were Type IB, and 21 patients were Type IIB. According to Sanders classification, 14 patients were Type I, 8 patients were Type IIA, 8 patients were Type IIB, and 5 patients were Type IIC. All the patients were treated with close reduction and percutaneous internal fixation by K-wires. The patients were regularly followed-up, and X-ray was taken at the follow-up to evaluate the bone healing. Results： The average operation time was 32.3 minutes （25 to 45 minutes）, and the average blood loss was 32.5 mL （20-40mL）. All the 35 patients were successfully followed-up, and the average follow-up time was 8.5 months （6-12 months）. All the patients got bone healing, and no skin necrosis and pin tract infection were found. Conclusion： Close reduction and percutaneous internal fixation by K-wires for calcaneal fractures have many advantages, such as short operation time, little blood loss, small operation injury and few complications.
