鼻内镜下鼻中隔畸形成形术患者围术期应激状态变化的研究

摘　　要：目的探讨鼻内镜下鼻中隔畸形成形术围术期应激状态的变化。方法选取2010年6月至2013年3月于攀枝花市第二人民医院耳鼻咽喉科进行鼻内镜下鼻中隔畸形成形术治疗的45例患者作为观察组,同期采用传统鼻中隔黏膜下矫正术治疗的45例患者作为对照组,比较两组患者治疗前后血清应激激素与氧化应激的变化。结果术后1 d与3 d观察组血清应激激素各指标水平均低于对照组,且两组患者术后1 d各指标水平高于术前及术后3 d,其呈现先升高后降低的趋势(P<0.05);观察组术后1 d与3 d血清氧化应激各指标水平均高于对照组,且两组患者各指标水平均低于术前(P<0.05)。结论鼻内镜下鼻中隔畸形成形术患者围术期应激状态变化小于传统手术,临床应用价值高于传统手术。Objective To study the change situation of stress state of patients with endoscopic nasal septum malformation septoplasty during perioperative period.Methods A total of 45 patients with endoscopic nasal septum malformation treated with septoplasty in Panzhihua City Second People＇s Hospital from Jun.2010 to Mar.2013 were selected as observation group,45 cases treated with traditional submucous nasal septum surgery during the same period were selected as control group,and the serum stress hormone and oxidative stress of the two groups before and after treatment were compared.Results 1 d and 3 d after operation the stress hormones indicators serum levels of the observation group were lower than the control group,and level of each index at the postoperative 1 d was higher than that of the preoperative and postoperative 3 d,which formed an up and then down trend（P 〈 0.05） ; serum level of each oxidative stress indicator at postoperative 1 d and 3 d of the observation group was higher than the control group,and each indicator of both groups were all higher than before surgery（P 〈 0.05）.Conclusion The change of stress state of patients with endoscopic nasal septum malformation septoplasty during perioperative period is smaller than that of the traditional operation,and its clinical application value is higher than that of the traditional operation.
