摘 要:目的:探讨高血压心肌肥厚时心肌细胞刚度变化及与心律失常的可能联系。方法:急性分离不同周龄(12周龄/21周龄)自发性高血压大鼠(spontaneously hypertensive rat,SHR)与正常同源常压大鼠(wistar-Kyoto,WKY)的心室肌细胞,利用原子力显微镜采集细胞微区力曲线,获取杨氏模量信息,进行统计分析。结果:12周龄及21周龄的SHR心室肌细胞杨氏模量均明显高于同龄正常对照组(4.61 kPa±2.08 kPa vs.3.55 kPa±1.35 kPa,5.95 kPa±1.98kPa vs.3.79 kPa±1.66kPa,P<0.01)。21周龄与12周龄SHR心室肌细胞杨氏模量比较,前者显著升高(5.95 kPa±1.98 kPa vs.4.61 kPa±2.08 kPa,P<0.01),而21周龄与12周龄WKY心室肌细胞杨氏模量比较则无明显变化(3.79 kPa±1.66 kPa vs.3.55 kPa±1.35 kPa,P>0.05)。结论:高血压发生心肌肥厚时心室肌细胞力学性质发生改变,即杨氏模量变大,细胞刚度增加,并且随着心肌肥厚严重程度逐渐加剧,可能参与心律失常的发生和维持。Objective:To investigate the stiffness change of hypertension-associated hypertrophied cardiomyocytes and the arrhythmogenic mechanism at micro scales.Methods:(1) Separating respectively ventricular myocytes from spontaneously hypertensive rats(SHR) and wistar-Kyoto rats (WKY) at different ages; (2) collecting force distance curves with atomic force microscope; (3) comparing the Young's moduli.Results:For 12-week-old and 21-week-old SHR,the Young' s moduli of ventricular myocytes were significantly larger than that from WKY of the same age (4.61 kPa ±2.08 kPa vs.3.55 kPa ± 1.35 kPa,5.95 kPa ± 1.98 kPa vs.3.79 kPa ± 1.66 kPa,P 〈 0.01).Comparing the Young' s moduli of ventricular myocytes from SHR of 21-week-old and 12-week-old,the former were higher than the latter (5.95 kPa ± 1.98 kPa vs.4.61 kPa ± 2.08 kPa,P 〈 0.01).However,the Young' s moduli of ventricular myocytes had not visible difference between WKY of 21-week-old and 12-week-old (3.79 kPa ± 1.66 kPa vs.3.55 kPa ± 1.35 kPa,P 〉 0.05).Conclusions:The intrinsic mechanical property of hypertension-associated hypertrophied ventricular myocytes has changed.Young' s moduli increased,in another word,hypertrophied ventricular myocytes were stiffer.In the meanwhile,as the degree of cardiac hypertrophy deteriorated gradually,the stiffness of ventricular myocytes increased accordingly.These changes were probably due to the occurrence and maintenance of cardiac arrhythmias.
