HIV/AIDS合并肛周尖锐湿疣9例临床分析及光动力联合电灼治疗疗效观察

摘　　要：目的分析HIV/AIDS合并复发性肛周尖锐湿疣的临床特征。探讨5-氨基酮戊酸光动力疗法联合电灼治疗HIV/AIDS合并复发性肛周尖锐湿疣的临床疗效。方法系统分析9例HIV/AIDS合并复发性肛周尖锐湿疣的临床资料。先用电灼去除肉眼可见的尖锐湿疣疣体,再进行光动力治疗。治疗后6个月判定疗效。结果 9例均为男男同性恋者,同性伴侣数7~1 167个,平均141个,其中4例合并梅毒。该组患者CD4+T细胞计数46~460个/μL,平均275个/μL。尖锐湿疣病程3个月~1.5年不等,平均1.2年;复发4~10次,平均5次,皮损数目7~37个,平均14个。经光动力联合电灼治疗,8例治愈,1例复发。结论男男同性恋者易患肛周尖锐湿疣,对反复复发、皮损面积广泛的男性肛周尖锐湿疣患者,需警惕同时合并艾滋病、梅毒等性传播疾病。对常规方法疗效不佳的HIV/AIDS合并肛周尖锐湿疣患者,光动力疗法联合电灼是比较有效的方法。Objective To study clinical characteristical of HIV/AIDS patients with recurrent perianal warts and discuss clinical efficacy of 5-aminolevulinic acid photodynamic therapy combined with electrocautery treating for HIV/AIDS patients with recurrent perianal warts. Methods To analyze systematically the clinical data of 9 HIV/AIDS patients with recurrent perianal warts. Firstly, removing visible warts of 9 HIV/AIDS cases with recurrent perianal warts with electrocautery, and then with photodynamie therapy, determining the efficacy after 6 months＇ treatment. Results Both of the 9 patients were MSM （men who have sex with men）, whose number of sexual partners were 7 - 1 167 ,with an average of 141. 4 cases were combined with sysphilis. The CD4 ~ T cell counts were 46 to 460 cells/p^L of this group, with an average of 275 cells/p.L. Course of the disease was ranging from 3 months to 1.5 years, with an average of 1.2 years. Recurrence of past anal con- dyloma was 4 to 10 times, with an average of 5 times. The number of the lesions was 7 to 37, with an average were 14. 8 cases were cured by photodynamic therapy combined with electrocautery treating, with one case of recurrence. Conclusion MSM were high risk people of perianal warts. For repeated recurrent, extensive areas of male anal lesions in patients with genital warts, combination of AIDS, syphilis and other sexually transmitted diseases need to be alert at the same time. Considering of poor efficacy of conventional methods, photodynamic therapy combined with electrocautery is a effective way for HIV/AIDS patients with recurrent perianal warts.
