瘢痕型皮肤结节病1例

摘　　要：患者女,57岁。面部陈旧瘢痕处红斑肿胀、变硬、触痛、微痒1个月。皮肤科情况:右额、眼睑、上唇人中沟原有瘢痕处淡红色浸润性肿胀隆起。下牙龈中部呈红色隆起结节。右眉弓皮损组织病理示:真皮内上皮样细胞构成的裸结节,境界清楚,周围散在淋巴细胞浸润,无干酪样坏死。陈旧性瘢痕出现活动表现为红肿、结节、触痛,可为诊断结节病的重要线索。A 55-year-old female presented with erythematous swelling, hardening, tenderness and mild itching over old scars on her face for one month. Physical examination showed reddish discoloration, infiltrative swelling, touch a tender spot over the former scars on the right forehead, upper eyelid, and philtrum. The other lesion appeared a red swollen tubercle in the middle of bottom gingiva. The histopathology of a biopsy from right brow revealed a dermal infiltration of nests （nated tubercle） composed of collections of epitheloid histiocytes which is non-case- ating epitheloid cell granulomas around scanty lymphocytes. The reactivation of old sears, which manifests as erythematous swelling nodules and tenderness, may better clue for the diagnosis of sarcoidosis.
