带血管蒂髂骨瓣移位治疗股骨颈骨折的回顾性研究

摘　　要：目的：比较联合带血管蒂髂骨瓣移植与单纯内固定术治疗青壮年股骨颈骨折的疗效。方法随机抽取本院2010年～2013年的股骨颈骨折病例80例，分两组。 A组为联合带血管蒂髂骨瓣移植组，B组为单纯空心钉内固定组，随访4个月～3年。比较术后拄拐下地、弃拐时间、骨折愈合时间、髋关节功能Harris评分及并发症发生率。结果术后下地活动及弃拐时间、骨折愈合时间A组明显短于B组；Harris评分A组优于B组。结论联合带血管蒂髂骨瓣移植组效果明显优于单纯内固定组，可缩短下地活动及骨折愈合时间，减少并发症。Objective To compare the curative effect of vascular pedicle iliac flap transplantation combined with internal fixation with simple internal fixation through retrospectively analyzing the cases of femoral neck fractures in young adults in traumatic orthopedics of the 89th hospital of PLA.Methods Forty cases of femoral neck fractures from 2010 to 2013 were randomly selected and divided to A and B group.A group consist of the cases of treatment with vascular pedicle iliac flap transplantation combined with cannulated screw internal fixa -tion,and B group consist of the cases of mere cannulated screw internal fixation .The periods of follow-up were 1 months -3 years.Then the postoperative ambulation time with crutches ,abandoning crutches time ,healing time ,Harris hip score and the incidence of complications were compared.Results The postoperative ambulation time and abandoning crutches time of group A were dramatically earlier than that of group B.The healing time of group A was obviously shorter than group B .Harris hip score of group A was better than group B .Conclusion The effect of vascular pedicle iliac flap transplantation combined with internal fixation is better than the control group .It can shorten ambulation time and healing time ,and it is beneficial to the recovery of hip function .
