六味补气胶囊对COPD急性加重患者Treg及Th1、Th2、Th17型细胞的影响

摘　　要：目的:观察六味补气胶囊对慢性阻塞性肺疾病(COPD)急性加重患者调节性T细胞(Treg)、辅助T细胞(Th)的影响。方法:将80例患者随机均分为治疗组和对照组,每组40例。治疗组:常规治疗基础上加服六味补气胶囊,每次口服3粒,每日2次;对照组:常规治疗基础上加服硝苯地平,每次10mg,每日3次,两组均30d为1个疗程,连服2个疗程。对COPD患者进行BODE评分、呼吸困难评分,采用肺功能仪观察COPD患者肺功能变化,采用酶联免疫吸附法检测血清γ-干扰素(IFN-γ)、白介素-4(IL-4)、白介素-17(IL-17)变化,采用流式细胞术检测外周血Treg表达。结果:与正常对照组比较,COPD患者IL-4、Treg显著降低,IFN-γ、IL-17显著升高(P<0.05,P<0.01);与治疗前比较,治疗组治疗后FEV1、FEV1/FVC、Treg、IL-4显著升高,呼吸困难评分、BODE评分、IFN-γ、IL-17显著降低(P<0.05,P<0.01)。与对照组治疗后比较,治疗组治疗后FEV1/FVC、Treg显著升高,BODE评分显著降低(P<0.05)。治疗组有效率显著高于对照组(P<0.05)。结论:六味补气胶囊通过上调COPD患者Treg、IL-4表达,下调IFN-γ、IL-17表达,调节Th1/Th2平衡,改善COPD急性加重症状。Abstract： Objective： To observe the effect of Liuwei Buqi Capsules on regulatory T cells （Treg）, helper T cell （Th） in patients with chronic obstructive pulmonary disease （COPD） of acute exacerbation. Methods： 80 patients were randomly divided into treatment group and control group, 40 cases in each group. Treatment groups： Based on the conventional therapy plus oral Liuwei Buqi Capsules, each with 3 capsules, 2 times a day; Control group： Based on the conventional treatment plus oral nifedipine, each with 10mg, 3 times a day, one course of treatment was 30 days, the treatment lasted for two courses. COPD patients were evaluated by BODE score, dyspnea score. Lung function was detected by Spirometer. 7-interferon （IFN-7）, interleukin-4 （IL-4） and interleukin-17 （IL-17） in serum were detected by enzyme-linked immunoassay （ELISA）. Treg of peripheral blood was detected by flow cytometry （FCM）. Results： Compared with normal group, IL-4 and Treg of peripheral blood were decreased, while the expression of IFN-~/, IL-1.7 in serum were increased in patients with COPD （P〈0.05, P〈0.0t）. After treatment, FEV1, FEV1/FVC, Treg, IL-4 were increased, and dyspnea score, BODE score, IFN-7, IL-17 were decreased in treatment group （P〈0.05, P〈0.01）. Compared with control group, FEV1/FVC and Treg were increased, BODE score was decreased in treatment group （P〈0.05）. The efficiency of treatment group was higher than that in the control group （P〈0.05）. Conclusion： Liuwei Buqi Capsules can improve COPD symptoms by up-regulating the expressions of Treg and IL-4, down- regulating the expressions of IFN-7 and IL-17, and regulating the balance of Th1/Th2.
