利胆化瘀汤联合熊去氧胆酸治疗原发性胆汁性肝硬化临床疗效观察

摘　　要：目的:观察利胆化瘀汤联合熊去氧胆酸治疗原发性胆汁性肝硬化患者的临床疗效。方法:将60例原发性胆汁性肝硬化患者随机分为2组,每组30例。对照组口服熊去氧胆酸(UDCA),治疗组在对照组的基础上加服利胆化瘀方,比较两组患者治疗前后淤胆、免疫指标的变化。结果:两组患者治疗后4周TBI、GGT、ALP较治疗前下降(P<0.05),但两组间比较无明显差异(P>0.05);两组患者治疗后12周TBI、GGT、ALP较治疗前明显下降(P<0.05),且治疗组疗效优于对照组(P<0.05)。两组患者治疗后4周IgG、IgM较治疗前未见明显下降(P>0.05),但治疗后12周IgG、IgM较治疗前下降(P<0.05),且治疗组比对照组更明显(P<0.05)。结论:利胆化瘀汤联合UDCA可明显改善原发性胆汁性肝硬化患者淤胆指标,疗效优于单用UDCA,可能与其调节免疫有关。Objective： To observe the clinical efficacy of ursodeoxycholic acid （ UDCA ）in combinationof Lidan Huayu Decoctioin in the treatment of patients with primary biliary cirrhosis （ PBC ）. Methods ： 60cases of patients with primary biliary cirrhosis were randomly divided into two groups. The control group wastreated with ursodeoxycholic acid. The treatment group added Lidan Huayu Decoction based on the controlgroup. We observed the changes of cholestasis indicators and immune parameters of patients with primarybiliary cirrhosis after the treatment. Result： After 4 weeks treatment, TBI, GGT and ALP were decreased intwo groups （ P〈0.05 ） but there was no significant difference between two groups （ P〉0.05 ）. After 12 weekstreatment, TBI, GGT and ALP were decreased in two groups （ P〈0.05 ） but more significant in the treatmentgroup than those of the control group （ P〈0.05 ）. After 4 weeks treatment, IgG and IgM didn＇t decrease intwo groups （ P〉0.05 ）. After 12 weeks treatment, IgG and IgM decreased in two groups （ P〈0.05 ）, but moresignificant in the treatment group than those in the control group （ P〈0.05 ）. Conclusion ： UDCA combinedwith Lidan Huayu Decoction can effectively treat PBC and improve the biomarkers. This may be related tothe immune regulation effect of Lidan Huayu Decoction.
