昆明市21家基层医疗机构基本药物制度实施成效与对策分析

摘　　要：目的:了解昆明市基层医疗机构基本药物制度实施成效及存在的关键问题,为进一步完善国家基本药物制度配套政策提供参考。方法:综合运用问卷调查、处方抽查、知情人访谈等方法,对2009年至2012年昆明市21家基层医疗机构基本药物配备、经济运行情况变化、社区居民及医务人员用药行为变化、门诊合理用药等进行实地调查并分析总结。结果:基本药物使用得到普及,但基本药物品种不能满足居民用药需求,基本药物供应无法充分保障;基本药物制度宣传培训力度不够;基本药物制度实施对经济运行情况并未产生明显影响,促进合理用药效果未体现。结论:应适度扩大基层医疗机构基本用药目录,完善基本药物招标采购及配送供应,加强基本药物制度宣传及培训,加大力度提高基层医疗机构诊疗及合理用药水平。Objective To understand the effect and the key problems of implementing essential drug system in basic medical institutions of Kunming,and provide reference for further perfecting the supporting policies of national essential drug system. Methods The basic drugs equipment,economic situation changes,the change of using drugs behavior by community residents and medical staff and outpatient rational drugs use in 2009- 2012 in 21 basic medical institutions of Kunming were investigated and analysised by comprehensive using questionnaire survey,prescription checks,known methods of interviews. Results Essential drugs were used widely,but the essential drugs could not meet the demand of residents,and the essential drugs could not be fully supplied; the essential medicine system advocacy training was not enough; the essential medicine system did not have a significant effect on the economic operation,and promoting the rational use of drugs was not reflected. Conclusion The essential drugs directory in basic medical institutions should be expanded appropriatly,the bidding procurement and distribution supply of the essential drugs should be improved,the propaganda and training of the essential drugs system will be strengthened; the level of diagnosis,treatment and rational drug use in basic medical institutions will be improved by intensified efforts.
