综合性医院扩大HIV咨询检测综合服务模式探索

摘　　要：目的探索医疗机构加大艾滋病病毒(HIV)阳性病例的发现,并减少阳性病例流失的有效方法。方法选定武汉3家大型综合性医院,培训、提高医生对HIV疑似病例的认知,主动提供HIV检测转介;引入社区组织进入医院,专门负责为医生转介的患者提供"一对一"匿名HIV检测、咨询服务,并配合医院疫情管理机构对初筛阳性者提供阳性确认、CD+4T淋巴细胞检测转介等服务。结果初步建立了综合性医院扩大HIV检测咨询综合服务模式;增加了HIV阳性病例的发现,在项目周期内(6个月)3家医院共检测2751名HIV疑似病例,检出HIV抗体阳性25例,全部转介至疾病预防控制部门完成阳性确证检测,无流失。结论在综合性医院开展扩大HIV检测咨询综合服务模式,能够有效扩大HIV阳性病例的发现,减少阳性病例的流失,有助于提高医疗机构参与艾滋病防治工作的社会责任感和临床医生参与艾滋病防治工作重要性的认识。Objective To explore effective methods for identification of more HIV/AIDS positive cases and re duction of withdrawal of HIV positive cases. Methods Three large general hospitals were chosen in Wu Han. Phy sicians were trained to improve their identification of HIV suspected cases, providing HIV testing referrals initiative ly. Community organizations were introduced into hospitals to provide ＂oneonone＂ anonymous HIV counselling and testing services, and cooperate with hospital epidemic management institutions to provide HIV positive confir mation, CD4 T testing and referral to the CDC. Results Comprehensive service model of scaling up HIV counseling and testing was preliminarily established in general hospitals. More HIV/AIDS positive cases were identified. Dur ing the project period （6 months）, a total of 2751 HIV suspected cases were screened and 25 were HIV positive. All the positive cases were referred to the CDC without loss. Conclusion The integrated service model of scaling up HIV counseling and testing in general hospitals could expand identification of HIV positive cases effectively, and re duee loss of HIV positive cases. It would be beneficial to improve social responsibility of medical institutions and recognition of clinicians on the importance of HIV/AIDS prevention and control.
