三磷酸肌醇受体在妊娠期高血压病患者胎盘组织中的表达及临床意义

摘　　要：目的探讨三磷酸肌醇受体(IP3R)在妊娠期高血压病(GHD)产妇胎盘组织的表达及临床意义。方法行剖宫产分娩的GHD产妇300例分为轻度子痫前期(A组,218例)和重度子痫前期(B组,82例);同期住院行剖宫产分娩的正常妊娠产妇200例作为对照(C)组。以免疫组化SP法检测产妇胎盘组织的IP3R表达。结果在正常妊娠和子痫前期患者胎盘组织中均有不同程度的IP3R表达,以滋养细胞及蜕膜组织中分布较多,绒毛血管内皮细胞及部分间质细胞液偶见其表达。A组和B组产妇胎盘绒毛滋养细胞IP3R表达强度均高于C组(强阳性检出率:44.5%和67.1%vs.15.0%),B组高于A组(P<0.05)。A组和B组产妇蜕膜组织中IP3R表达强度均高于C组(强阳性检出率:42.7%和62.3%vs.13.5%),B组高于A组(P<0.05)。结论在GHD产妇胎盘组织中,IP3R表达上调,提示其可能参与了GHD的发生及发展过程。Objective To investigate the expression and clinical significance of three phosphoinositide receptor（IP3R） in the placenta of puerpera with gestational hypertension disease （GHD）. Methods With immunohistochemical SP method, the expression intensity of IP3R in the placental tissues was detected in 218 puerperas with mild preeclampsia（group A）, 82 puerperas with severe preeclampsia（group B） and 200 puerperas with normal pregnancy（group C）. All puerperas underwent Cesarean section delivery. Results IP3R was expressed in different degrees in the placental tissues from groups of A, B and C, which mainly expressed in the sertoli cells and decidual tissues but occasionally seen in the vascular endothelial ceils and part of interstitial cells. In the villi nourish ceils of the placenta in groups of A and B, the strong positive detection rate of IP3R was higher than that in group C（44. 5% and 67.1% vs. 15.0%）（P〈0. 05） ,which was higher in group B than that in group A（P〈0. 05）. So did the strong positive detection rate of IP3R in the decidual tissues（42.7% and 62.3% vs. 13.5%）（P〈0. 05）. Conclusion The expression of IP3R in the placenta of puerpera with GHD is up-trgulated indicating that the IP3R expression may participate in the occurrence and development process of GHD.
