股骨近端解剖锁定钢板与联合加压交锁髓内钉治疗股骨转子间骨折的疗效比较

摘　　要：目的：探讨股骨近端锁定解剖钢板（PF-LCP）与联合加压交锁髓内钉（InterTan）治疗股骨转子间骨折的临床疗效。方法38例股骨转子间骨折患者中，使用PF-LCP治疗20例，使用InterTan治疗18例。对两组手术及术后恢复情况进行分析。结果术中出血量：InterTan 组为（150.5±18.2）ml，PF-LCP 组为（262.2±22.3）ml，差异有统计学意义（P＜0.05）。手术时间和术中透视时间：PF-LCP 组为（82.55±8.32）min 和（7.56±2.56）s，InterTan组为（96.28±9.05）min和（18.10±5.34）s，两项差异均有统计学意义（P＜0.05）。38例均获得随访，时间6～18个月。患者骨折均获得愈合。骨折愈合时间：PF-LCP组（15.1±6.5）周，Inter-Tan组（14.5±3.8）周；术后髋关节Harris评分：PF-LCP组（83.5±9.5）分，InterTan组（81.1±10.8）分；两项差异均无统计学意义（P＞0.05）。结论 PF-LCP与InterTan都是治疗股骨转子间骨折较为理想的内固定方法，相对于InterTan治疗PF-LCP治疗出血量较多，但手术时间及术中透视时间较短。Objective To investigate the curative effect of therapy for intertrochanteric fracture between the proximal femoral locking compression plate （PF-LCP）and InterTan.Methods 38 patients of intertrochanteric fractures were divided into two groups：20 cases with the fixation of PF-LCP,18 cases with the fixation of InterTan.The information on surgery and postoperative recovery conditions of the two groups were analyzed.Results Intra-operative blood loss was （150.5 ±18.2）ml in the InterTan group and （262.2 ±22.3）ml in the PF-LCP group,this difference had statis-tically significant（P〈0.05）.Operation time and X-ray exposure time were （96.28 ±9.05）min,（18.10 ±5.34）s in the InterTan group,（82.55 ±8.32）min,（7.56 ±2.56）s in the PF-LCP group,these differences were statistically significant（P〈0.05）. 38 cases were followed up for 6-18 months after operation,fractures in all the patients were healed.Fracture healing time was （14.5 ±3.8）weeks in the InterTan group and （15.1 ±6.5）weeks in the PF-LCP group.Harris score of hip joint was （81.1 ±10.8）points in the InterTan group and （83.5 ±9.5）points in the PF-LCP group.There was no statistically significant deference between the two groups （P〉0.05 ）.Conclusions PF-LCP and InterTan are both the ideal fixation of therapy for intertrochanteric fracture.Treatment with PF-LCP has a much intra-operative blood loss in operation,but a shorter operation time and X-ray exposure time than treatment with InterTan.
