输精管结扎后附睾淤积和精子肉芽肿形成机理探讨

摘　　要：本文观察了输精管结扎后1年内家兔睾丸、附睾体积和组织学变化。结果是对照组活体测量睾丸体积为3.36±0.54ml,尸检为1.39±1.59ml;术后3和6个月活体测睾丸体积分别为4.24±0.81ml和4.40±1.27ml,术后10个月处死组尸检体积为3.24±1.84ml,12个月为2.39±0.61ml,分别与对照组比较有显著差异(P(0.01);对照组尸检附睾尾体积为0.51±0.21ml,结扎10和12个月组分别为2.36±1.32ml和1.79±1.15ml,与对照组比较差异显著(P<0.01)。光镜下见曲细精管不同程度萎缩,炎细胞浸润,附睾管扩张,附睾精子肉芽肿只见于精索静脉结扎兔中。Twenty-four rabbits were bilaterally vasectlmized and observed for one year in com- parison with 25 normal control male rabbits. Besides histopathological examination,the volume of the testes and epididymides was measured. The results showed that the average volume of the testes was 3. 36±0. 54 ml in biopsy and 1. 39±0. 59 ml in postmortem exam in the control,while in the experimental group the volume was 4. 24 ± 0. 81ml 3 months after vasectomy, 4. 40 ± 1. 27ml 16 months after vasectomy in biopsy and 3. 24±1. 84ml 10 months and 2. 40±0. 61ml 12 months in postmortem exam. The average volume of the epididymides in postmortem exam, was 0. 51 ± 0. 20ml in the control,2. 36±1. 32ml 10 months and 1. 79±1.15ml 12 months after vasectomy. By comparison,the volumes in the vasectomized were significantly larger than those in the control. Under light microscope,the main changes in the testes wore atrophy of the seminiferous tubules and infiltration of the inflammatory cells into the interstitial tissue. The main changes in the epididymides were expansion and atrophy of the epididymal tubules. Spermatic granuloma in the epididymides were found only in 4 rabbits after ligation of the spermatic veins in the vasectomized animals .
