摘　　要：目的 :用组织多普勒成像 (TDI )与二尖瓣口血流多普勒频谱评价正常人左室舒张功能 ,比较两种方法的临床意义。方法 :正常成年人 16 1例 (2 1～ 70岁 ) ,按年龄段每 10岁 1组共分为 5组。采用 HP Sonos 5 5 0 0彩色超声仪 ,S4超宽频变频探头 ,于心尖四腔心切面上取左室外侧壁近瓣环处 TDI频谱 ,测量收缩期、舒张早、晚期最大峰值速度及速度时间积分 (s- Vmax、e- Vmax、e- TVI、a- Vmax、a- TVI)。二尖瓣口血流频谱舒张早、晚期最大峰值速度及速度时间积分 (E- Vmax、E- TVI、A- Vmax、A-TVI) ,计算各自比值。结果 :随着年龄增长 ,e、E各项测值逐渐降低 ,a、A各项测值逐渐上升。各年龄组相互对比 ,除速度时间积分外 ,每间隔一年龄组有显著差异。两种方法存在相关关系 ,以 e- Vmax/ a- Vmax与 E- Vmax/ A- Vmax相关性最好。结论 :TDI和二尖瓣口舒张期血流频谱两种技术均能反映左室舒张功能 ,以 e- Vmax/ a- Vmax、E- Vmax/ A- Vmax比值意义最大。Objective: To evaluate left ventricular diastolic function in normal people by tissue Doppler imaging(TDI) and mitral orifice flow spectrum, and compare the clinical significance of 2 methods. Methods: One hundred and sixty one normal adults(age 21 70 years) were divided into 5 groups by every 10 years and detected by HP Sonos 5500 with S4 probe. The maximal velocity and time velocity integral of systole and early or late diastole (s V max , e V max , e TVI, a V max , a TVI) of left ventricular lateral wall TDI spectra were measured and compared with the maximal velocity and time velocity integral of mitral orifice flow spectra (E V max , E TVI, A V max , A TVI). Results: With age increasing the values of e,E diminished, and the values of a,A increased. There was a correlation between 2 methods, among these the correlation of e V max /a V max and E V max / A V max was the highest. Conclusion: Both TDI and mitral orifice flow spectrum can assess left ventricular diastolic function, and the significance of e V max /a V max and E V max / A V max are the most important. [
