2型糖尿病胰岛素抵抗、血脂紊乱与脂肪肝关系的研究

摘　　要：目的　探讨 2型糖尿病 (T2DM)胰岛素抵抗 (IR)、血脂紊乱与脂肪肝的关系 .方法　对T2DM并脂肪肝患者进行血脂水平、空腹血糖 (FBG)及血浆胰岛素 (FINS)的测定 ,计算胰岛素抵抗指数 (IRI)、体重指数 (BMI) ,并与T2DM无脂肪肝患者作对照 .结果　T2DM并脂肪肝组与无脂肪肝组相比较 ,BMI、甘油三酯 (TG)、低密度脂蛋白 (LDL)、FINS、IRI均升高 (p <0 .0 5 ) .结论　T2DM并脂肪肝患者存在明显脂代谢紊乱、胰岛素抵抗及超重 .对T2DM尽可能防止高胰岛素血症 ,控制血糖 ,维持正常脂质代谢 ,减轻IR 。Objective To study and analyse of relationship between insulin resistance, the disturbance of blood lipid and fatty liver in patients with Type 2 diabetes mellitus(T2DM).Methods We measured the levels of blood lipid,fasting blood sugar and blood insulin in patients of T2DM with fatty liver ,calculated the insulin resistance index,body mass index,and compared them with those of patients with non-fatty liver. Results The former's body mass index,triglyceride(TG),low density lipoprotein(LDL),fasting blood insulin, and insulin resistance index were significantly higher than that of the latter.Conclusions The patients of T2DM with fatty liver have obvious lipid metabolic disturbance,insulin resistance and overweight.For them,we should protect the patients from hyperinsulinemia,control their blood sugar,maintain their normal lipid metabolism and reduce insulin resistance.We think these treatment are very important for protecting and reducing the occurrence of the fatty liver in T2DM patients.
