摘 要:目的 探讨心脏停搏供体肝脏切取与修整的方法。 方法 1 999年 1 2月至 2 0 0 0年 1 1月共施行原位肝移植 7例 ,供肝的切取采用原位灌注 ,快速供肝切取的方法 ,联合切取供体肝 7个 ,供肾 1 4个。 结果 供肝热缺血时间平均为 4.5min ,7例移植肝恢复血循环后平均 1 2min即有金黄色胆汁泌出 ,除 1例术后死于脑出血外 ,其余 6例均获得成功 ,最长已存活 1 80天。 结论 原位低温灌注、肝肾联合快速切取法缩短了热缺血时间 ,有效地避免供体器官损伤 ,保证了供体器官的质量。Objective To investigate the methods for the procuring of donor liver from cadaver. Methods From December 1999 to November 2000, 7 cases of orthotopic liver transplantation (OLT) had been performed. Donor livers were procured with rapid in situ cool perfusion technique. Seven donor livers and 14 donor kidneys were obtained by combined procuring. Results The warm ischemia time was 4.5 min on the average. Normal bile was drained out 12 min after restoration of the liver ciculation. One case died of intracerebral hemorrhage postoperatively, and 6 cases survived up to now, the longest survival was 180 days. Conclusion The method of rapid in situ cool perfusion technique contracts the warm ischemia time and effectively avoids the reperfusion injury of donor organ, so that it ensure a good quality of the donar liver.
