摘 要:目的 探讨新生儿中解脲脲原体 (ureaplasmaurealyticum ,UU)感染的临床相关因素。方法 随机选择 189例新生儿病房住院新生儿 ,应用培养和聚合酶链反应方法检测其咽部分泌物 ,同时用酶联免疫吸附试验检测新生儿血清UU IgM。详细记录临床资料。结果 住院新生儿咽部分泌物UU阳性率 2 5 9% ,其中早产儿 31 3% ,足月儿 2 4 1% (P >0 0 5 ) ;小于胎龄儿 2 9 7% ,适于胎龄儿2 5 0 % (P >0 0 5 )。新生儿肺炎患儿咽部分泌物UU阳性率为 38 5 % ,非感染性疾病患儿为 2 3 9%(P >0 0 5 )。胸部X线片异常者咽部分泌物阳性率为 31 8% ,正常者为 2 4 1% (P >0 0 5 )。血清UU IgM阳性率为 34 4% ,其中早产儿为 39 6 % ,足月儿为 32 6 % (P >0 0 5 ) ;小于胎龄儿为 48 6 % ,适于胎龄儿 30 9% (P <0 0 5 )。新生儿肺炎患儿血UU IgM阳性率为 5 3 8% ,非感染性疾病患儿为31 3% (P <0 0 2 5 ) ;X线胸片异常者UU IgM阳性率为 5 0 0 % ,正常者为 2 9 7% (P <0 0 2 5 )。 结论住院新生儿中小于胎龄儿UU感染率高于适于胎龄儿。UU感染与新生儿肺炎和胸部X线片异常者密切相关。Objective To determine the clinical correlation factors between the ureaplasma urealyticum (UU) infection and the hospitalized neonates Methods One hundred and eighty nine hospitalized neonates were selected randomly The pharyngeal secretions were examined by UU cultivation and PCR amplification, and serum UU IgM were detected by ELISA The detailed clinical data had been collected and recorded Results The positive detection rate of UU in pharyngeal secretions among hospitalized neonates was 25 9%, of which 31 3% was in premature infants, 24 1% in full term infants( P >0 05), 29 7% in SGA and 25.0% in AGA infants ( P >0 05) The positive detection rates of pharyngeal excretion in neonates with pneumonia and neonates without pneumonia were 38 5% and 23 9%, respectively( P >0 05), and in neonates with abnormal and normal chest X ray findings were 31 8% and 24 1%, respectively( P >0 05) The positive rate of UU IgM was 34 4%, of which 39 6% was in premature infants and 32 6% in full term infants( P >0 05);48 6% in SGA and 30 9% in AGA infants( P <0 05) The positive rates of UU IgM in neonates with pneumonia and neonates without pneumonia were 53 8% and 31 3%, respectively( P <0 025), in neonates with abnormal and normal chest X ray findings were 50% and 29 7%, respectively( P <0 025) Conclusion Among the hospitalized neonates the UU infection rate was higher in SGA than in AGA The UU infection showed significant correlations to the neonatal pneumonia and the abnormal chest X ray findings
