摘 要:目的探讨新生儿吸入性肺炎和肺出血与单纯疱疹病毒(herpes simplex virus,HSV)感染的关系.方法采用实时荧光定量聚合酶链反应的方法(fluorescence quantitative polymerase chain reaction,FQ-PCR)检测26例因吸入性肺炎和19例因肺出血死亡的新生儿肺组织中HSV-DNV.结果45例肺组织中,共检出HSV-DNA 19例,总检出率为42.2%(19/45),其中26例吸入性肺炎肺组织中检出10例,检出率为38.5%(10/26),19例肺出血肺组织中检出9例,检出率为47.4%(10/19);吸入性肺炎与肺出血组的HSV-DNA定量值分别为(3.65±7.24)×105与(8.26±2.27)×106,2者比较差异无显著性(P>0.05);HSV-DNA定量值大于1.0×105基因拷贝/毫克与小于1.0×105基因拷贝/毫克比较,前者临床症状重,死亡时间早.结论在新生儿吸入性肺炎和肺出血致死因素中,应注意HSV感染的作用.对HSV定量值大于1.0×105基因拷贝/毫克的患儿,加强抗病毒的治疗可能是必要的.Objective To investigate the relation of herpes simplex virus (HSV) infection to neonatal aspiration pneumonia and pulmonal hemorrhage. Methods HSV-DNA in pulmonal tissues of 26 neonates with neonatal aspiration pneumonia and 19 neonates with pulmonal hemorrhage was detected with fluorescence quantitative polymerase chain reaction (FQ-PCR). Results In pulmonal tissues of 45 neonates, HSV-DNA was detected in 19 neonates, the positive rate was 42. 2 % (19/45). In 26 neonates with aspiration pneumonia, HSV-DNA was detected in 10 neonates, the positive rate was 38. 5 % (10/26). In 19 neonates with pulmonal hemorrhage, HSV-DNA was detected in 9 neonates, the positive rate was 47. 4 % (10/19). The means of HSV-DNA in neonates with aspiration pneumonia and in neonates with pulmonal hemorrhage were (3. 65 ± 7.24) × 105 copies/mg and (8. 26 ± 2. 27) × 106 copies/mg, respectively, with no significant difference between the two groups (P>0. 05). In 1 9 neonates with HSV infection, the neonates whose HSV-DNA was more than 1. 0 × 105 copies/mg had heavier clinical symptoms and died earlier than those whose HSV-DNA was less than 1. 0× 105 copies/mg. Conclusions In the cause of death of neonatal aspiration pneumonia and pulmonal hemorrhage, we should pay attention to HSV infection. It may be necessary to strengthen antiviral treatment.
