TIPPS术前后直肠门脉闪烁显像门体分流率测定的实验研究

摘　　要：目的　探讨直肠门脉 99m Tc O- 4 闪烁显像在 TIPPS术中支架口径的选择和判断术后分流道的开存情况中的价值。方法　制备老龄犬门脉高压动物模型 ,并经颈动脉插管后行 TIPSS术 ,在 TIPSS术前后分别进行直肠 99m Tc O- 4 门静脉闪烁显像 ,图像加合后在门静脉、肝脏及心脏部位勾画感兴趣区 ,计算门体分流率及门静脉区计数同肝区及心区计数之和的比值 (P/ HL)。结果　所选支架口径与门体分流率之间存在明显正相关(P<0 .0 5) ;行 TIPSS术后分流道开放时 ,P/ HL与正常动物无明显差异 ,而分流道关闭时 ,PI/ HL显著低于正常 (P<0 .0 5)。结论　 TIPPS前后直肠 99m Tc O- 4? Objective To discuss the usefulness of per rectum portal vein scintigraphy in selection of stent diameter and judgement of the opening and closing of shunt tract.Methods Canine portal vein hypertention model was prepared and TIPPS was administered through catheterization in jugular vein.Per rectum portal vein scintigraphy was performed before and after TIPPS.Images were added and ROIs were drawn on portal vein,liver and heart.Then shunt index and ratio of counts of portal vein to that of liver and heart(P/HL) were calculated.Results The diameter of shunt stent was relate positively to shunt rate( P <0 05) with relative coeffiecient 0 895.After TIPPS,P/HL have no significant difference with that of normal animal when the shunt tract was opened( P <0 05) and was significantly lower than that of normal animal when the shunt tract closed( P <0 05).Conclusions Per rectum portal vein scintigraphy was helpful in selection of stent diameter and judgement of opening and closing of shunt tract. 〔
