痹痛消治疗颈痛症疗效观察及其对血液流变学的影响

摘　　要：为探讨中药痹痛消治疗神经根型颈椎病对其疼痛和血液流变学的影响 ,以颈复康冲剂作对照 ,将以疼痛为主要症状的 6 0例神经根型颈椎病患者随机分为两组进行治疗观察 ,测定治疗前后疼痛分值和血液流变学状况。结果显示治疗后两组疼痛分值均明显减少 ,两组间治疗前后疼痛积分的差值比较有显著性差异 (t=1.943P<0 .0 5 ) ,治疗组疼痛的缓解优于对照组。治疗前两组病人的全血高、中、低切变率和血浆粘度、红细胞最大变形指数及红细胞最大聚集指数均明显增高 ,血液呈明显的高粘滞状态。治疗后 ,治疗组各项血液流变学指标均有改善 ,血浆粘度改善最为明显 (经统计学处理 ,t=2 .34,P<0 .0 5 )。而对照组治疗后血液流变学指标均无明显变化。表明颈椎病病人的确存在明显的血液流变学异常 ,中药痹痛消治疗颈痛的镇痛作用与血液流变学、特别是血浆粘度改善有关。In order to explore the diagnostic value of the ultrasonic examination on cervical vertigo of the middle aged and old,detect the vertebral arteries of145cases of cervical spondylopathy with vertigo( observed group) , which were compared with the normal values of30 cases of healthy persons( normal group) ,simultaneously apply neck- rotating test to36cases of cervical vertigo.The results showed:1In the observed group there is56% incurvation of vertebral artery which obviously increased,especially in C4～ 5and C5～ 6;2 In the internal diameter( D) ,velocity of blood flow of the peak value in the systole( Max) and velocity of blood flow in late diastolic phase( Min) of the vertebral artery,the observed group had a significant difference from the normal group;3 In the neck- rotating test,the contraction peak and blood flow rate of the late diastolic phase in the vertebral artery were significantly lowered( P<0 .0 5,P<0 .0 1) ,and pulsation index ( PI) and resistance index( RI) were obviously higher than those in the lying position( P<0 .0 5,P<0 .0 1) .It was suggested that structure changes such as incurvation and stenosis of the vertebral artery caused by spondylopathy were reasons of cervical vertigo.These provided objective evidence for diagnosing cervical vertigo by color Doppler ultrasonic examination.
