摘  要:目的 研究罗哌卡因加和不加肾上腺素用于臂丛麻醉的药代动力学特征。方法  18例择期骨科下肢手术病人 (ASAⅠ~Ⅱ级 ) ,分成两组 ,Ⅰ组为不加肾上腺素组 (n =9) ,Ⅱ组加肾上腺素 1:2 0 0 ,0 0 0 (n =9) ,0 75 %罗哌卡因 2 0ml加生理盐水 10ml,共 30ml,肌间沟径路行臂丛麻醉 ,操作在 1min内完成。分别在给药前及给药后 10、2 0、30、4 5、6 0、90min、3、4、6、9、12h采取颈内静脉血 ,用高效液相色谱仪测定罗哌卡因血药浓度 ,用药代动力学计算程序 (3P97)统计处理。结果 两组血药浓度峰值 (Cmax)、达峰值时间 (tmax)及血药浓度曲线下面积 (AUC)比较均没有统计学差异 (P >0 0 5 )。结论 加肾上腺素并不能改变罗哌卡因用于臂丛麻醉的药代动力学参数。Objective To determine the pharmacokinetics of ropivacaine used with or without epinephrine for brachial plexus block Methods Eighteen ASA Ⅰ Ⅱ adult patients undergoing elective orthopaedic surgery on upper limb received a single injection of 30ml of 0 5% ropivacaine for brachial plexus block The patients were divided randomly into two groups One group (n=9) received 0 5% ropivacaine 30 ml (150mg) without epinephrine while the other group (n=9) received 0 5 % ropivacaine 30ml with epinephrine 1:200000 Blood samples were taken from internal jugular vein, which was cannulated at 10,20,30,45,60,90 min,3,4,6,9, and 12h after drug administration for determination of plasma ropivacaine concentration by high performance liquid chromatography The pharmacokinetic parameters were calculated using a 3P97 computer program Results There were no significant differences in peak plasma concentration of ropivacaine (Cmax) and time to peak (tmax) and the mean area under the concentration time curve (AUC) between both groups Conclusions The addition of epinephrine does not alter the pharmacokinetics of ropivacaine when used for brachial plexus block
