摘　　要：目的　正确认识腰骶部筋膜脂肪疝 ,提出临床区域分型和术中分型 ,并探讨治疗方法。方法　对 139例腰骶部筋膜脂肪疝患者进行手术治疗 ,术中详细观察脂肪疝疝出部位及病变情况。结果用自行拟定的疗效评定标准评定 ,术后优良率达 89%。结论　腰骶部筋膜脂肪疝是腰骶部筋膜下脂肪组织通过深浅筋膜破裂口或裂隙孔疝出 ,直接卡压或机化后形成粘连带压迫局部周围组织或皮神经所产生的一系列腰腿痛症状和体征 ;按疝发区域可分为髂腰型和骶髂型 ;按术中情况可分为单纯型和粘连型 。ObjectiveTo study the clinical typing of lumbarsa cral lipocele and to discuss its surgica l treatment.Methods139 patients with lipocele in fassia of lumbosacral region were treated surgical ly.The herniation area and pathological changes of lipoceles were carefully obse rved during the operation.ResultsThe postoperative results revealed excel lent and good rates in 89%.ConclusionThe mechanism of this disease is apparan tly due to the prominent adipose tissues under the lumbosacral fascia projecting through the split or hiatus of the deep and superficial fassia resulting in com pression of the cutaneous nerves locally ,or the adipose tissues organizes and fo rms adhesion causing compression of the local tissues and cutaneous nerves.Lumba r and leg pains are the main symptoms.Ac cording to prominent locations,the disea se can be classified as iliac lumbar type and sacro iliac type;and the intraoperat ive findings showed the conditions of th e simple and adhesive varieties.Early op eration is the effective treatment metho d for this condition.
