摘　　要：目的　提出窝滑膜囊肿的命名及分型 ,探讨手术适应证及治疗方法。方法　自 1992至1998年手术治疗 190例窝滑膜囊肿患者 ,分别采取术前检查 ,术中所见进一步了解发病部位及囊肿是否与膝关节相通而进行临床分型。应用半膜肌或腓肠肌腱膜瓣修补交通型和混合型囊肿 15 0例。单纯彻底切除滑囊治疗单纯型囊肿 40例。结果　有 12 5例随防 0 5～ 5年 ,用自行拟订的疗效评定标准 ,优 10 0例 ,良好 2 0例 ,好转 5例。术前按窝滑膜囊肿部位 ,分为内侧型及外侧型 ;按术中情况分为单纯型、交通型及混合型。结论　早期采取半膜肌或腓肠肌腱膜瓣修补和单纯彻底切除滑囊的手术方法治疗不同类型滑膜囊肿 ,无一例复发 。ObjectiveTo study the nomenclature and classifica tion of synovial cyst of popliteal fossa ,to explore its treatment method and ind ication of operation.Methods190 patients with synovial cyst of popli teal fossa were treated with operation.T he disease was classificed according to pathogenic position and whether the cyst was communicating with the knee joint;t h ese were decided by preoperative observa tion in combination with intraoperative findings.150 cases of communicating type and mixed type were repaired by semimem branous muscle or aponeurosis of gestroc nemius.40 cases of simple type were trea ted with simply resection of the synovia l bursa.ResultsThe follow up period of 120 cases ranged from 0 5 year to 5 years.Excellent resu lts were obtained in 100 cases,good in 2 0 and fair in 5.Before operation,the dis ease was classified as medial and latera l types according to the location of the lesion.The intraoperative findings indi cated that the disease could be classifi ed as simple type,communicating type and mixed type.ConclusionRepairing the cyst after resection with semimembranous muscle or aponeurosis of gestrocnemius and simple resection of t he synovial membrane completely are the best methods for the treatment of differ ent types of popliteal cysts.
