摘  要:为研究全胃肠外营养 (TPN)对患坏死性小肠结肠炎 (NEC)的新生儿免疫功能的影响 ,本文于 1 996年 1月~ 1 998年 1 2月 2 3例新生儿NEC进行了研究 ,测定应用TPN前、TPN3天、7天时静脉血中补体C3、C4 、B因子、转铁蛋白 (Tr)和外周血T细胞亚群的变化。结果显示 ( 1 )补体C3、B因子、Tr在应用TPN3天、7天时均较应用TPN前增高 (P <0 .0 5 ) ;( 2 )外周血T细胞亚群测定显示CD4 升高 (P <0 .0 5 ) ,CD8下降 (P <0 .0 5 ) ,CD4 /CD8比值升高 (P <0 .0 5 )。表明TPN在一周内可以通过对补体系统。To explore the influenees of tofal parenteral nutri tion (TPN) on immune function of infants with necrotizing enterocolitis (NEC). Using 23 cases NEC were studied during January 1996 to December 1998. To evaluate the immune function of the infants with NEC, Complements (C3, C4)、Factor B、 Transferrin (Tr) and T cell subsets (CD4,CD8) were examined at different time: before TPN,3 days later, 7 days later. Results Shoued (1)Complement C3, Factor B and Transferrin incresased at the third day and seventh day during TPN (P<0.05). (2) Tcell analysis showed that induce helper cell (CD4) increased while suppressor cell (CD8) decreased (P<0.05). Conclusion Thesofose, the immune function of the neonates with NEC was augmented during TPN by affecting compliment system、 protein metabolism and the T ell differentiation. The possible mechanics were based on: (1) TPN can accelerate the synthesis of complement C3、 factor B and transferrin. (2)Poly non saturated fatty acid can suppress the excreting of prostategladin and stimulate the T cell proliferation
