彩超诊断下肢深静脉瓣膜功能的临床研究

摘　　要：目的　应用CDFI判断下肢大隐静脉曲张患者的深静脉瓣膜功能。方法　检查 40例患者的髂外、股总、股浅和静脉。股静脉瓣膜功能做Valsalva试验 ,静脉瓣膜功能行挤压远端小腿放松后 ,以CDFI观察血流颜色及频谱有无返向 ,并测量返流的持续时间。结果　有返流者管腔内径明显增宽 ,最大流速加快 ,两者相比均有显著性差异 (P<0 .0 1) ,与经大隐静脉穿刺顺行性静脉造影对照 ,超声诊断符合率 87%。结论　CDFI诊断股静脉返流的敏感性和特异性高 ,并能判断返流程度。Objective To detect the deep venous valve function of the patients with extremity venous engorgement(LEVE).Method The external iliac、common femoral、superficial and poplitel veins of 40 cases of LEVE were studied by color Doppler flow image(LEVE).After Valsalva test, femoral venous valve function was observed. The distant small legs were oppressed and relaxed for observing poplitel venous valve function,CDFI was used to detect the change of blood flow color and the spectrum direction and measure the lasting time of regurgitants.Result Compared with angiography,the diagnosis positive rates of CDFI is 87%.The infernal diameter and maximal blood flow velocity of the regurgitant cases are larger then regurgitant cases(P<0.01)。Conclusion The sensitivity and specificity of CDFI to demonstrate the exact of regurgitant of deep venous valve is high.CDFI also could diagnose the severve degrees of regurgitant of deep venous valve.
