摘 要:目的 :探讨胃食管电极心室起搏抢救心脏性猝死的临床应用。方法 :以 18例急重症缓慢型心律失常引起的心脏性猝死 ,进行 18例次胃食管电极心室起搏。结果 :16例 (88.9% )起搏成功。其中 15例 (83.3 % )起搏脉宽 40msec ,阈值 2 2 .1± 8.7V ;1例脉宽 10msec ,阈值 32V。结论 :体会有 3点优点 :①心室夺获率高 ,阈值电压低 ;②操作非创伤性、安全、方便、快速 ;③不需中断心肺复苏术 (CPR)的持续进行。Objective: To study the clinical role of ventriculer pacing with gastroesophageal electrode in cardiac sudden death. Methods: 18 patients with cardiac sudden death of acute slow arrhythmia were treated with ventriculer pacing by the gastroesophageal electrode. Results: 16(88.9?%) patients were successfully paced, the pulse-widths were 40?msec in 15(83.3?%) patients with thresholds of voltage 22.1±8.7?V including the pulse-width 10?msec in one patient with threshold of votage 32?V. Conclusion: The advantages of this method are (1)the rate of ventriculer seizing is high, and the threshold of voltage is low. (2) the procedure is safe?convenient?quick and non-invasive. (3)no interruption of cardiac-lung push resuscitation (CPR).
