紫外线照射充氧自血回输对慢性乙型肝炎患者肝功能及血清可溶性细胞间粘附分子-1水平的影响

摘　　要：目的　探讨紫外线照射充氧自血回输 (UBIO)对慢性乙型肝炎患者肝功能及血清可溶性细胞间粘附分子 - 1(sICAM - 1)含量的影响及其意义。方法　 16 8例慢性乙型肝炎患者分为UBIO治疗组 86例和对照组 82例 ,观察两组的疗效和血清sICAM - 1的含量变化。结果　两组患者治疗后血清SB、ALT、AST均有不同程度下降 ,但UBIO治疗组下降更明显 ,与对照治疗组比较 ,差异有显著性 (t=2 .5 76 ,2 .76 3,2 .746 ,P <0 .0 5 ) ;SB、ALT和AST复常率UBIO治疗组分别为 82 .6 % ,5 5 .8%和 46 .5 % ,对照治疗组分别为 39.0 % ,30 .5 %和 18.3%。两组SB、ALT和AST复常率比较 ,差异均有非常显著性 (t=33.5 4,10 .95 ,15 .18,P <0 .0 1)。UBIO组血清sICAM - 1水平比治疗前明显下降 (t=3.175 ,P <0 .0 1) ,但对照治疗组血清sICAM - 1治疗前后无显著性差异 (t=0 .6 6 1,P >0 .0 5 )。结论　UBIO治疗可能会下调肝内sICAM - 1的表达 ,促进肝功能的恢复。Objective To study the effect of ultraviolet blood irradiation and oxygenation(UBIO) on liver function and serum soluble intercellular adhesion molecule-1(sICAM-1) level in chronic hepatitis B and its significance.Methods 168 patients with chronic hepatitis B were randomly allocated into two groups:86 cases,based on conventional drug treatment,were received the UBIO therapy(UBIO group).Other 82 cases were only treated with conventional drug treatment(control group).The serum biochemical parameters and sICAM-1 level were evaluated in two groups.Results The levels of serum bilirubin(SB),alanine transaminase(ALT) and aspartic transaminase(AST) all were reduced in two groups,especially in the UBIO group was more obvious.Compared with the control group,there was significant difference(P<0.05).The normalized rate of SB,ALT and AST in UBIO group was 82.6%,55.8%,and 46.5% respectively,but that of control group was 39.0%,30.5% and 18.3%; there was obvious difference in two groups(P<0.01).After the UBIO therapy,the level of sICAM-1 in UBIO group was obviously reduced(P<0.01),that in control group unchanged(P>0.05).Conclusion It is suggested that the UBIO may suppress the expression of sICAM-1 in liver and can promote the improvment of liver function.
