云南白药对稳定期COPD痰液细胞因子调控的研究

摘　　要：应用云南白药胶囊治疗稳定期慢性阻塞性肺疾病 (COPD)患者 ,观察对其气道细胞因子的调控作用 ,探讨其在COPD中的疗效及机理 .方法 :6 0例COPD患者随机分为对照组 (安慰剂A组 ) ,治疗组 (云南白药B组 )各 30例 ,采用诱导排痰、化学发光免疫检测技术、单盲试验对比观察药物对气道细胞因子 :白细胞介素 (IL) - 1β ,IL - 6 ,IL - 8,肿瘤坏死因子 (TNF -a)等参数含量的变化 ,同时观察肺功能第 1s用力呼气容积实测值 /预计值百分比 (FEV1% )、出凝血指标、血小板计数 (PLT)、纤维蛋白原 (PF)、出凝血时间 (PPT)及肝肾功能等副作用 .结果 :B组与A组比 ,治疗后气道细胞因子含量参数相应值均有显著性降低 (P <0 0 1) ,FEV 1%有明显改善 (P <0 0 5 ) .结论 :缓解期COPD患者气道炎症与IL - 1β ,IL - 6 ,IL - 8和TNF -a释放增多有关 .云南白药治疗后可调节细胞因子生成 ,抑制气道炎症作用 ,副作用小 ,有临床应用价值 .To observe the effect of oral Yunnan Baiyao capsule regulating the cytokines in induced sputum of patents in stable chronic obstructive pulmonary disease (COPD), to explore its beneficial effect on COPD, a single-blind placebo-controlled trail, 60 cases of CM were divided into placebo-controlled M for A 30) and treated group (group B 30), before and after the administration of Yunnan Baiyao, the cytokines in induced sputum (IL-1β,IL-6, IL-8,TNF-a) were determined with chemiluminescence immunoassay (CLIA). and the forced expiratory volume in l second (FEV l,) by pneumometer, platelet count(PLT), plasma prothrombin time (PPT), plasma fibrinogen (PF) also were detected to observe the drugs influence on function of liver and kidney during the treatment.The level of cytokines was decreased significantly (P<0 01),and FEV1 was much improved (P<0 05)after the administration in group B, PPT was shorter but P>(0 05) after treatment. The airway inflammation of patients in stable COPD was related to the increased IL-l β, IL-6.IL-8 and TNF-a, Yunnan Baiyao had a effect on regula ting cytokines and depressing airway inflammation with fewer side-effect. It is avaluable drug for stable COPD.
