皮神经营养血管蒂岛状皮瓣修复手足烧伤创面的临床观察

摘　　要：目的　探讨皮神经营养血管蒂岛状皮瓣修复烧伤创面的临床效果。方法　采用带前臂外侧皮神经、尺神经腕背支、腓肠神经、腓浅神经营养血管蒂的逆行岛状皮瓣修复手足部位烧伤后软组织缺损创面 10例。皮瓣面积最大 11cm× 10cm ,最小 5cm× 4cm。结果　 9例皮瓣完全成活 ,1例部分成活。随访 6～ 17个月 ,皮瓣质地优良 ,外观及功能满意。结论　此类皮瓣血供充分 ,不牺牲主干血管 ,切取容易 ,旋转弧度大 ,能够满意地修复手足附近烧伤后软组织缺损创面。Objectives To report the results of neurocutaneous axial island flaps on the limbs in the clinical application.Methods 10 patients with soft tissues defects around the hand and foot after burnt were treated with reserve flow island flaps based on the lateral forearm cutaneous nerve (2 cases),wrist-dorsal branches of the ruler nerve(1 case),sural nerve (5 cases),peroneus superficialis nerve(2 cases ).The flaps measured 11cm×10cm to 5cm×4cm. Results The flaps totally survied in 9 patients ,the other was partially survived .the color and function were satisfactory after 6～17 months follow-up.Conclusions These flaps have the reliable blood supply and the character of avoidance of sacrificing the major arteries.And they are easy to dissection and bave a large arc of rotation. That were satisfactory in repairing the soft tissue defects around the hand and foot after burnt.
