P-ERG在糖尿病性视网膜病变诊断和治疗中的应用研究

摘　　要：本实验对正常人 2 0例 (4 0眼 )和 6 4例糖尿病患者 (12 8眼 ,其中无视网膜病者 6 2眼 ,单纯型视网膜病变者 5 8眼 ,增殖型视网膜病变者 8眼 ) ,进行了图形视网膜电图 (P- ERG)检测。并对进行光凝治疗的 31眼在治疗前当日 ,治疗后 7天、 14天、 30天、 6 0天时分别测定 P- ERG。结果显示 ,在糖尿病组中 ,当眼底尚未出现改变时 ,P- ERG各参数值已有变化 ,b波波幅降低 ,潜伏期延长 (P<0 .0 1)。随着糖尿病性视网膜病变 (DR)的发展 ,变化更加显著 ,组间差异逐渐加大。P- ERG的 b波波幅和潜伏期分别与糖尿病病程显著相关。光凝治疗后 7天 ,P- ERG的 b波波幅出现明显但暂时的降低 ,随后逐渐恢复到治疗前水平。b波潜伏期在治疗前后无明显变化。结果揭示 P- ERG在 DR的早期诊断方面具有高度敏感性 ,通过检测糖尿病者的 P- ERG可以预测 DR的发生、发展 ,估计 DR的严重程度。同时 P- ERG还可以做为对接受光凝治疗的The pattern electroretinogram (P-ERG) were examined in 20 normal individuals (40 eyes) and 64 patients (128 eyes) with diabetes mellious (DM). We also recorded the P-ERG at 5 different time points in 31 eyes of patients who had argon laser photocoagulation, i.e.: before treatment, and 7, 14, 30, and 80 days after laser treatment. The results showed that P-ERG had already changed in the DM patients whose ocular fungus had no changes. With the development of diabetic retinopathy (DR), the P-ERG showed further impairments, and the differences between groups increased gradually. The changes in P-ERG were related to the diabetic courses. There was an obvious but temporary depression of P-ERG amplitude at 1 week post-photocoagulation, then the value returned to baseline gradually. The b-wave latency remained constant after the treatment. 'The results suggested that P-ERG has high sensitivity in the early diagnosis of DR and it also can be used to predict the progress and extent of the retinopathy.
