足背复合皮瓣移植修复手背皮肤及肌腱缺损

摘　　要：目的:为一期修复手背皮肤和伸指肌腱,尽快恢复手的功能。方法:采用吻合血管带伸趾肌腱足背复合皮瓣修复6例。其中带3根肌腱1例,2根肌腱3例,1根肌腱2例,均切取Ⅲ、Ⅳ区伸趾长肌腱。结果:皮瓣全部成活,随访5个月～1年半,除1例环指、1例中指伸指不全,经二次手术,缩短肌腱后恢复满意外,其余均一期恢复。结论:该方法具有一期同时修复软组织和肌腱缺损,疗程短;保留肌腱的血供,愈合快、粘连少,手功能恢复快,外形满意等特点。本文对肌腱选择、缝合固定张力及手术注意事项进行了讨论。Objective: To repair defect of skin and extensor tendon of hand in one stage and recover hand function quickly. Method: 6 cases are treated by the composiste free flap of pedal dorsum with long extensor tendon of the toes locating in Ⅲ or Ⅳ areas. Result: All flaps survived completely. All cases were followed up for 6-18monthes. The results were satisfactory. Unsatisfactory function recovery of extending finger was only found in one case of the third finger and the fourth finger. After tendon was shortened secondarily, function recovery was satisfactory. Conclusion: The authors discussed merits and demerits of this method. Meanwhile, choice of long extensor tendon of toes and tension of sewing up and key points of the operation were discussed.
