Relationship between Serum Arylesterase and Serum Lipid,Lipoprotein and Apoprotein in Patients with Renal Disease and Its Clinical Significance

摘  要:目的:研究慢性肾疾病患者血清芳香酯酶(ArE)活性变化与血脂、脂蛋白、载脂蛋白A_1(Apo A_1)、载脂蛋白B_(100)(Apo B_(100))的关系及其临床意义。方法:通过检测慢性肾炎(CHN)、肾病综合征(NS)和慢性肾功能衰竭(CHF)等病人血清ArE活性、胆固醇(Tc)、高密度脂蛋白胆固醇(HDLc)及其亚类胆固醇(HDL_2c与HDL_3c)的含量,进行统计分析。结果:血清ArE活性:NS患者[(0.200±0.032)u/ml]与CHF[(0.186±0.042)u/ml]低于对照组[(0.260±0.019)u/ml],尿毒症患者[(0.165±0.042)u/ml]降低更为明显,CHN[(0.229±0.033)u/ml]稍有减低。血清ArE/Tc与ArE/HDL_3c:NS分别为0.037±0.003与0.225±0.080,CHN为0.04±0.004与0.217±0.050,均低于对照组(0.065±0.004,0.310±0.100)。血清ArE活性与HDLc、HDL_2c、ApoA_1、白蛋白及内生肌酐清除率(Ccr)呈正相关,r分别为0.39、0.52、0.55、0.40。ArE/Tc和HDLc/Tc、HDL_2c/Tc、Apo A_1/Apo B_(100)的相关系数依次为0.61、0.69、0.59,均呈正相关。而ArE与血清肌酐(r=-0.36)则为负相关。结论:血清ArE与脂代谢具有密切关系,检测血清ArE活性在防治肾疾病脂代谢紊乱并心血管疾病和了解肾功能损害具有一定的临床意义。Objective:To study the relationship between arylesterase (ArE) activity and serum lipid, lipoprotein, apoprotein A1 (Apo-A1 ) and apoprotein B100(Apo-B100 ) in patients with renal disease and its clinical significance. Methods: The ArE activity,total cholesterol (Tc) ,high density lipoprotein-cholesterol (HDLc) and its subclass cholesterol (HDL2c,HDL3c),Apo-A1 and Apo-B100 were measured in patients with chronic nephritis (CHN), nephropathy syndrome (NS), chronic renal failure (CHF) and in normal persons. Results:The values of serum ArE activity in NS [(0.200±0.032)u/ml] ,in CHF [(0.186± 0.042)u/ml] and in CHN [ (0.299 ± 0.033)u/ml] groups were significantly lower than those in control group [ (0.260 ±0.019)u/ml],among which the value of serum ArE activity in uremia group [(0.165±0.042)u/ml] was the lowest. The ratios of ArE/Tc and ArE/HDL3c inNS (0.225± 0.080 and0.036±0.003) and in CHN (0.040±0.004 and 0.217 ±0.050) were lower than those in control group (0.065±0.004 and 0.310 ± 0.100). The ArE activity was positively correlative with HDLc, HDL2c and Alb, its correlative coefficients ( γ) were 0.39,0.52,0.30 and 0.55 respectively. There was a positive correlation between ArE/Tc and HDLc/Tc, HDL2c/Tc, Apo-A1/Apo-B100 , the γ were 0.61,0.69, and 0.59 respectively. Conclusion:There was a close rektionship between the ArE activity and lipid metabolism. The serum ArE activity is a good diagnostic index for patients with renal disease.
