摘　　要：目的采用心电图明尼苏达编码方法分析城市高血压患者的心电图特点 ,为高血压伴心脏损害的患者提供心电图诊断的可能依据。方法在上海南市区对 34 45名 35岁及以上的确诊原发性高血压患者进行心电图检查 ,编码和分析心电图及其相关因素。结果研究对象中近 1/ 3有心电图编码异常 (编码异常率为 30 .97% ) ,主要异常和次要异常编码的频率分别为 14.2 8%和 16 .6 9%。主要编码异常、次要编码异常率和大部分单项编码率均随着年龄的增加而上升。单项编码以 T波改变和 ST段压低最为多见 (分别为 10 .5 1和 7.6 3% )。男性室内传导阻滞、电轴偏移和 R波高电压高于女性 ,而女性 4和 5编码 (ST- T改变 )多于男性。除年龄、性别外 ,单因素分析结果显示收缩压、脉压与心电图编码异常有关。结论在高血压人群中 ,心电图主要异常编码率较高 ,以 T波改变和 ST段压低多见 ,因此应重视和控制该人群的血压 。Objective Using Minnesota code to analyze the electrocardiographic characteristics in hypertensive patients.Methods Data from Shanghai hypertensive patients(aged 35 years and older)who participated in the Hypertension Prevention and Control Program were used to determine prevalences and associated variables of different abnormalities on the resting electrocardiogram.Electrocardiographic abnormalities were coded according to the Minnesota classification system.Results About one third of the subjects showed one or more abnormalities on their electrocardiogram.Major abnormalities and minor abnormalities occurred in 14 28% and in 16 69% respectively.Most prevalences were positively related with age.Minnesota codes 5 (T wave changes) and 4 (ST abnormalities)were found to be the most prevalent(10 51% and 7 63% respectively) in this population and were especially higher in women than in men,while code 2(axis deviation),code 3(high amplitude R waves) and code 7(ventricular blocks) were quite common in men.Systolic blood pressure and pulse pressure showed positive correlation with major and minor abnormalities.Conclusion Since the major abnormalities of ECG were relative high and ST T changes were common,we should prevent and control the high blood pressure and related target organs damage in this population.
