摘 要:目的 对比有无脑梗死的两组病例的颈动脉斑块的超声特点 ,了解不同回声斑块声学密度定量(AD)值和病理组成、斑块内不同回声区域面积的大小在引发脑梗死方面的危险性 ,提供测定斑块性质和危险性的半定量指标。方法 应用彩色多普勒超声系统、AD技术和病理学方法 ,检测有脑梗死 (Ⅰ组 ,5 6例 )和无脑梗死 (Ⅱ组 ,5 0例 )患者的颈动脉斑块不同回声区的AD值、病理成分及斑块不同回声区域面积占整个斑块面积的百分比。结果 ①弱回声斑块AD值为 ( 2 2 .36± 3 .0 9)dB ,以脂质沉积为主 ;低回声斑块AD值 ( 34.70± 4.15 )dB ,以纤维化为主的强回声斑块AD值 ( 4 6 .2 9± 4.0 7)dB ,以钙盐沉积为主。②两组颈动脉斑块内弱回声区域与强回声区域面积的百分比之间的差异有显著性意义。结论 应用超声和AD技术可以了解颈动脉斑块的性质 ,评价存在颈动脉斑块患者有无发生脑梗死的危险性。Objective To imvestigate the ultrasonic properties of carotid plaque in cerebral infarction,the pathological components and AD value of plaque with different ultrasonic property,the risk of different lucent area in plaque leading cerebral infarction,and to provide some new semi quantitative parameter for plaque components property and risk.Methods With color Doppler flow imaging system,the pathological components,the percentage of each kind of echo area in the total plaque and AD value of 65 carotid plaque in 56 cases with cerebral infarction and 50 cases of non infarctions were measured.Results The AD value of weak echo plaque is ( 22.36 ± 3.09 )dB,which mainly composed of lipid deposit,( 34.70 ± 4.55 ) dB of low echo plaque,which mainly composed of fibrosis deposit,( 46.29 ± 5.06 ) dB of high intensity echo,which mainly composed of calcium deposit.There is a great difference between the area percentage of weak and high intensity echo area in both groups.Conclusions AD technology can evaluate the property of carotid plaque and the risk of carotid plaque in cerebral infarction.
