摘  要:目的 :对大鼠肝移植模型进行技术改进 ,并观察将SD大鼠肝脏移植至Wistar大鼠时免疫排斥发生的规律。方法 :将“二袖套法”肝移植技术在袖套管制作 ,受体麻醉 ,肝上下腔静脉、门静脉及肝下下腔静脉吻合等方面进行改进 ,行供肝取自SD大鼠的Wistar大鼠肝移植 30例 ,并随机分为环孢霉素 (CsA ,2mg/kg ,术后第 1天至第 7天皮下注射 )治疗组和对照组 ,观察移植术后免疫排斥情况。结果 :肝上下腔静脉吻合时间为 6 .5~ 10 .0min ,无肝期为 8.5~ 12 .0min ,93%以上的移植大鼠生存超过 7天。对照组大鼠术后 9~ 13天全部死亡 ,组织病理学提示有典型的免疫排斥 ,而CsA治疗组到目前为止生存良好。结论 :对大鼠肝移植技术进行改良后可明显提高移植大鼠的生存率 ,SD大鼠移植至Wistar大鼠的肝移植组合为高排异组合 ,是较理想的研究肝移植排斥及移植免疫耐受的动物模型。Objective:To study and reform the technique of orthotopic liver transplantation in rat, and to observe the allograft rejection of liver transplantation in Wistar rat operated. Methods:Orthotopic liver transplantation in 30 Wistar rats which received SD liver were established by using 'the two-cuff technique' with some modifications in cuff tube, recipients anaesthesia and the anastomoses of suprahepatic vena cava, portal vein, infrahepatic vena cava. The thirty rats after transplantation were randomly divided into control (n=5) and therapy group (n=15), and the latter was subcutaneously injected by cyclosporine 2 mg/kg daily for 1 week after operation. In the meantime, the allografts rejection of all transplantation rats were studies. Results:The time of vascular anastomosis for suprahepatic vena cava was 6.5 to 10 minutes, the anhepatic period was 8.5 to 12 minutes, more than 93% of the grafted animals survived for seven days. The control group all died 9 to 13 days after transplantation, and their histopathologic examination showed typical allografts rejection. Up to now, the cyclosporine therapy group are surviving. Conclusion:The modified technique can increase survival rate of liver transplantation in rat. The donor liver of SD rats and recipient Wistar rats combination is a high rejection model. It can be effectively used to study allograft rejection and liver transplantation tolerance.
