摘 要:目的 研究吸烟对骨密度的影响。方法 采用现况研究法 ,对 386例 (吸烟者 15 7例、非吸烟者 2 2 9例 )中国北方汉族男性人群进行骨密度测量和问卷调查 ,逐步回归方法分析主要影响因素 ,用方差分析和协方差分析法了解吸烟对骨密度的影响。结果 各部位骨密度值随吸烟量的增加及吸烟年限的延长而下降。日吸烟量超过 15支者 ,腰椎、股骨颈、Ward三角区和全身骨密度值为1 12 7、0 95 2、0 844和 1 148g/cm2 ,均显著低于日吸烟量未超过 15支者 (1 179、1 0 0 2、0 90 7和 1 2 19g/cm2 ) ,其中股骨颈和Ward三角区骨密度也分别显著低于非吸烟者 (0 998和 0 896g/cm2 ) ;修正体重等混杂因素后 ,这种差异在腰椎和全身部位仍然显著。同时吸烟 15年以上者 ,其骨密度值比日吸烟量少于 15支且吸烟不足 15年者低 4 6 %~ 6 7%。Objective To examine the effects of cigarette smoking on bone mineral density (BMD) in Chinese men. Methods BMD was measured by dual energy X ray absorptiometry and an interview was conducted with questionnaire for each of 386 Han ethnic male subjects in northern China by a cross sectional study design, including 157 smokers Major risk factors for BMD loss were analyzed by multiple regression and effects of smoking on BMD was evaluated with analysis of covariance. Results BMD in most sites of the body decreased with the increase in number and duration of cigarette smoking BMD in those who smoked more than 15 cigarettes per day were 1 127 g/cm 2, 0 952 g/cm 2, 0 844 g/cm 2 and 1 148 g/cm 2 in the lumbar spine, femoral neck, Ward′s triangular and the total body, respectively, significantly lower than those who smoked less 15 cigarettes per day (1 179 g/cm 2, 1 002 g/cm 2, 0 907 g/cm 2 and 1 219 g/cm 2, respectively), with P <0 05 BMD in the femoral neck and Ward′s triangular of the smokers was significantly lower than that of the non smokers (0 998 and 0 896 g/cm 2), with P <0 05 BMD in the lumbar spine and of the total body in the smokers was also significantly lower than that in the non smokers adjusted for confounding factors, such as body weight, etc BMD in those smoked more than 15 cigarettes per day for over 15 years decreased by 4 6% 6 7%, compared with those smoked less 15 cigarettes per day for less than 15 years. Conclusion Lower BMD was observed in the smokers, especially in heavy smokers
