摘 要:目的 :探讨星形胶质细胞瘤血管成分中血管内皮生长因子(VEGF)及其受体mRNA表达改变与瘤周水肿MRI的关系及肿瘤增殖能力对血管成分的影响 ,并探讨两者之间的关系。材料和方法 :采用半定量逆转录 -聚合酶链反应(RT -PCR)方法 ,对49例脑胶质瘤手术切除标本以及12例脑外伤内减压术中得到的正常脑组织检测了VEGF、mRNA表达水平。另外 ,用免疫组化SP法对胶质瘤及正常脑组织的微血管密度(WVD)并与星形胶质细胞瘤MRI、病理学资料进行分析。结果 :胶质瘤组织中的VEGF、mRNA表达水平明显增高 ,达65.3 % ,随着病理分级的提高其表达水平有升高趋势 ,HUVEC及3株胶质瘤细胞表达水平亦较高 ,而正常脑组织表达水平及表达率较低。胶质瘤中的微血管密度(MVD)与胶质瘤级别之间有等级相关性 ,VEGF、mRNA的表达同MVD呈正相关。同样 ,采用MRI评分方法 ,随着病理级别增加 ,MRI分值亦随之增加(p<0.01~0.001)。增强形态(CE -HET)、瘤周水肿、增强程度(CE -D)已被证明与病理级别良、恶性程度密切相关。经多元线性偏回归分析 p<0.001。结论 :VEGF的高表达同MVD对胶质瘤的恶性生物学行为的评估有重要意义 ,星形胶质细胞瘤的微血管成分改变对MRI增强程度、瘤周水肿、信号不均匀、增强形态有明显影响 ,血管丰富。Purpose:To study relationships between the changes of the expression of vascular endothelial growth factor(VEGF) as well as its receptors mRNA in gliomas,vacular components in astrolytoma and MRI features,and the influence of the proliferative activity of tumors on the vascular components.Materials and methods:The mRNA level of VEGF and its recptors in surgical samples of 49 patients suffering from glioma were studied using semi-quantitative reverse transcription polymerase chain reaction(RT-PCR).3 glioma cell lines,the human umbilical vein endothelial cells(HUVEC) and 12 samples of normal brain tissues obtained from craniocerebral injury surgery as control.Immunohistochemical SP method was used to analyse the intratumor microvessel density(MVD).Methods MRI,Immunohistochemical SP Method was used to analyse the intratumor microvessel density(MVD).Results:The expression level of VEGF,mRNA was significaney high in gliomas,the positive rate was 65.3%.The expression level paralleled with the grade of glioma.HUVEC and 3 glioma cell lines also expressed high levels of these factors,but the normal brain tissues had lower expression.The MVD increased with glioma pathologic grades.There was a positive correlation between mRNA level of VEGF and MVD.The MVD increased with glioma pathologic grades.MRI score increased with pathological grades(p<0.01~0.001) Contrast enhancement-degree(CE-D)peritumoral edema and contrast enhancement heterogeneity(CE-HET) thus proved to be related to the pathological grade by a multiple regression analysis(p<0.001).Conclusion:VEGF and mRNA have an important significance in evaluating the malignant biological behavior of glioma cells as well as MVD.The overexpression of VEGF resulted in vascular proliferation of gliomas and played a critical role in the invasion of tumor tissues.Chang of the vascular components of gliomas had obvious influences on peritumoral edema,CE-D and CE-HET.Tumors with rich vascular components were often accompanied by peritumoral edema and the tumors with enhanced CE-D
