43例肺科病人痰BACTEC法培养结核分支杆菌时污染菌种的分析

摘　　要：目的　探讨肺科病人痰BACTEC法培养污染菌种分布特点 ,以期更好地控制污染 ,降低污染率。方法　对 1997年 10月至 1999年 1月收治的 6 50例肺科病人痰标本BACTEC法培养污染标本 ,用常规普通细菌培养鉴定方法进行分离鉴定 ,并对结果进行分析。结果　 6 50例肺科病人痰BACTEC法培养标本中 ,污染 4 3例 ,总污染率为 6 .6 % (43/ 6 50 )。对 4 3例污染标本进行普通细菌培养鉴定 ,共分离出 12种菌株 ,其中 4例培养出 2～ 3种菌株 ,共得 4 8株。以枯草杆菌和微球菌为主 ,分别为 16株 (33.3% ) ,9株 (18.8% )。另外 ,还分离出金黄色葡萄球菌 6株 (12 .5% ) ,白色念珠菌 3株(6 .2 % )以及革兰阴性杆菌 14株 (2 9.2 % ) ,且分为 7种细菌。结论　 (1)实验室污染仍为主要污染源 ,应加强严格的无菌操作。 (2 )革兰阴性杆菌、金黄色葡萄球菌及白色念珠菌共占到 4 7.9% (2 3/4 8) ,提示与肺部感染及口咽部病原菌污染有关[1] ,建议临床加强肺部炎症控制。Objective To investigate the characteristics of the distribution of polluting bacteria in the sputum growing medium used for BACTEC which from pulmonary department patients,so we can do much better in controlling pollution and reduce the pollution ratio.Methods There were 650 sputum samples by means of BACTEC which were from pulmonary department patients between Oct.97 and Jan.99.To evaluate the polluted sputum samples in these 650 samples in normal way of bacterial culture and analyse the results.Results In the 650 samples,43 cases were polluted,total pullotion rate was 6.6%(43/650).To evaluate in normal way of bacterial culture,the 43 polluted samples and got 12 kinds of strain.There were 2～3 kinds of strain from every one of 4 samples.So 48 strain were got.Mainly of B.subticis and M.agilis.16(33.3%) and 9(18.8%) respectively.The other were 6(12.5%) strains of S.aureus;3(6.2%) strains of C.albicans and 14(29.2%) strains of gram-negative bacilli.The gram-negative bacilli can be divided into 7 kinds of bacterias.Conclusion (1)Laboratory pollution is the main pollution source.Non-bacteria operation must be strictly controlled.(2)The gram-negative bacilli,S.aureus and C.albicans were 47% (23/48).This may be related to the pulmonary infection and pathogenic bacteria in the mouth and pharynx [1].We suggest to increase the control of the pulmonary inflammation in clinic.
